    Provide the following information to generate your software license. The license file will be emailed to the address specified below. Please review your choices carefully as a fee may be charged to reissue the license. A Note about License Types: You may choose a Standalone or Network License. With a Standalone License, the software will only work on a single computer locally. If you have a Computer ID Key, you can move it and the corresponding Standalone License file from your work computer to your home computer or laptop. With a Network License, the software will work on a local area network, even if you only have a single license. You have the option of checking out a Network License from the License Manager Server to a laptop by using the commuter utilities WCommute and WRCommute. Please fax or e-mail this form to CSI. This form can also be found in the installed progam folder. Your license file will be sent to you via e-mail, so please ensure that the e-mail address you provide is correct.

    The license can be locked to a specific computer or to a Computer ID Key (CID). Refer to the Installation Instruction Card to determine the Locking Code. Note: a Vista Network license requires a CID key.
