RELEASE WAIVER AND QUITCLAIM

1. I am allowing _________________________________ (name of delegate) to join the ARC Young Leaders Camp, a three-day youth camp from February 14-16, 2013 to be held at the First Pacific Leadership Academy in Antipolo City. This youth camp is organized by the Asiawide Refreshments Corporation (ARC) and NGP Integrated Marketing Communications, Inc.

2. I understand that the organizers have taken the necessary steps to ensure the safety of all the participants in this three-day youth camp and for this reason, I hereby state, that I do hereby remiss, release and discharge ARC and all its sponsors and partners, including their respective officers, stockholders, directors, agents and employees, from any claim, including claims arising from and /or by reason of loss or damage to belongings or bodily injuries in the event of an accident;

3. I further state that the contents of this Release Waiver and Quitclaim was explained to me in a language known and understood by me and I hereby confirm that all the allegations herein are true and correct to the best of my knowledge and abilities.
