Who Qualifies?
Adults, age 19-64 No minor children at home(if you have minor children, you may qualify for other coverage) No income or low income: o Approximately $450 a month if single person, $600 a month for a married couple (higher income if working) U.S. citizen or legal resident for 5 years

Why Apply for Adult Medical Program (Plan A)?

Pays for basic medical care Low or no co-pays More coverage than County Health Plan B Will be automatically Eligible for Full Medicaid on 1/1/2014

Use DHS application form 1171 which is available at local DHS offices Sign and date application Mail or drop off completed application Or online at www.mibridges.michigan.gov/access/ Click Apply for benefits Follow on line instructions If you need help filling out this application, DHS must help you.
