This letter will serve as a formal DEMAND notice to you requesting the following documents within 30 days of receipt of this notice:

1) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all State & Federal Income Tax Returns for years 2004 to 2013. Including all refunds collected by Sylvester Johnson as well as joint returns with Mary CadeiJohnson. 2) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all Wells Fargo Bank personal and/or joint checking and/or savings account statements for Sylvester Johnson from 1/1/2004 to 3/14/ 2013. 3) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all Provident Credit Union Statements from 1/1/2004 to 3/14/2013 for Sylvester Johnson joint checking and/or savings account with Mary Cadei-Johnson. 4) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all Retirement Savings Accounts for Sylvester Johnson from Sunnyvale Department of Public Service and/or 401k from 1/1/2004 to 3/14/2013. 5) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all bank statements for Sylvester Johnson and Mary Cadei- Johnson including any/all documentation how and who provided the down payment of $175,000.00 for the purchase of 267 Brookview Dr Tracy, California 95377. 6) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all Countrywide Home Loan 1st mortgage payments regarding 267 Brookview Dr, Tracy, CA 95377. Including any/all bank statements for Sylvester Johnson and/or Mary Cadei-Johnson showing who paid home loan and how/where and who obtained the funds.

7) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all information regarding the $100,000.00 DiTech Home Equity Line of Credit (2nd mortgage ) information including any/all cancelled checks regarding: 267 Brookview Dr, Tracy, CA 95377 from 1/1/2004 to 12/31/2009. Including itemized list of financial documentation and how the home equity line of credit was obtained and distributed including names and addresses and phone numbers to verify all accounts for all monies spent. Including all cancelled checks and payments made by Sylvester Johnson and/or Mary Cadei-Johnson including final payment and satisfactory Note that Lien was paid in full. 8) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all letters and notices regarding foreclosure of 267 Brookview Dr, Tracy, California 95377. 9) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all notarized and certified copies of contract regarding the Post Office Box located in Sunnyvale, California from 1/1/2004 to 3/14/2013 giving any/all permission and/or authorization to collect United States Postal Service mail for Mary Cadei-Johnson and family.

10) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide a certified, itemized and notarized copy/copies of any/all proof and documentation of personal assets from any/all savings, checking, retirement, 401K distributions, disability, Social Security income prior to marriage to Mary Cadei-Johnson. 11) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide itemized financial statements, tax returns regarding sale of 2560 Meadowbrook Lane, Tracy, CA 95376 including all property improvements, costs and documentation of personal assets spent on home improvements, mortgage payments, personal loan payments including any/all rents collected from Queenie Linderman prior to and during marriage to Mary Cadei-Johnson including proceeds from sale of 2560 Meadowbrook Lane, Tracy, California 95376. 12) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide all contracts of sale and tax returns of 2560 Meadowbrook Lane, Tracy, CA 93576 prior to and during marriage.

13) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide notarized and certified copy of Quit Claim Deed for 2560 Meadowbrook Lane Tracy, CA 95376 obtained during marriage to Mary Cadei-Johnson. 14) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all employment paycheck stubs including an itemized list of deductions for the pay period of 1/1/2004 to 3/14/2013. 15) I, Mary Cadei-Johnson DEMAND Sylvester Johnson to provide any/all Student Loan information regarding Alyssa Cobbs including all payments made by Sylvester Johnson and/or Mary Cadei-Johnson. Including any/all satisfactory paid notices. I, Mary Cadei-Johnson has requested the above information from Sylvester Johnson since the Dissolution of Marriage on November 6, 2006. Sylvester Johnson repeatedly denied Mary Cadei-Johnsons request. I, Mary Cadei-Johnson has requested the above information from the City of Sunnyvale, California since 2006 and has been repeatedly denied. Since November 2006, I, Mary Cadei-Johnson attempted numerous times to retrieve my personal mail including State and Federal Income Tax Returns from Sylvester Johnson, United States Postal Service, Internal Revenue Service, CountryWide Home Loans, DiTech Home Loan including information regarding the PO Box located in Sunnyvale, CA that Sylvester Johnson had obtained prior to or during the divorce proceedings for himself and Mary CadeiJohnson without knowledge or consent. All requested have been repeatedly denied.

Lastly, I want to thank you for a quick resolution regarding this matter. However, if you are unwilling to comply I will have no other option but, to file a lawsuit and take this matter to Court. Sincerely,
