A 52 years old man comes to ER with crushing substernal chest pressure, radiating to the jaw and left shoulder.

He felt a lot of gas in his stomach, dyspnea, nausea and had vomited two times at home. He does not smoke but he does not exercise either. Physical examination: height 165 cm, weight 72 kg pulse 84 x/min regular. BP: 130/80 mmHg. No murmurs on chest auscultation, lung was clear, and there is no abnormalities found on abdomen.
