Board Act of 1974 to investigate transportation accidents, determine tie probabie causes
of the accidents, issue safety recommendations, study transportation safety issues, and
evaluate the safety effectiveness of goverment agencies involved in transportation. The
Safety Board makes public its actions and decisions through accident reports, safety
studies, special investigation reports, safety recommendations, and statistical reviews.

Abstract: This report explains the collision of USAir flight 1493 anc Skywest flight 5569 on
a runway at the Los Angeles Intemational Airport on February 1, 1891. The safety issues
discussed in the report are air traffic management and equipment at the airport: aircraft
ior lighting and canspieti tional awaranass during takeoff and landing and
nS On airport surfaces controller workload, performance, and supervision;
transport accident survivability, evacuation standards and procedures, intericr
fumishing flammability standards, and survival devices. Recommendations conceming
these issues were made to the Federal Aviation Administration.
