    NAME: MAYER , RAYMART DARIGDIG
    AGE: 7 YRS SEX: MALE ADDRESS: 219C. SITIO G. CAFMON , MALABON CITY DATE OF BIRTH: 06/03/2009 FIRST VISIT AT OPD: 02/14/2011

    CHIEF COMPLAIN: Inward deviation of Left eye since 6 years. CLINICAL HISTORY: 6 years PTC, patient mother noticed inward deviation of the left eye, 50% of the day. No consultations done, no medications taken. 4 years PTC, patient mother noticed deviation at all times.2 years PTC, patient mother noticed shallowing of left nasal labial fold. No consultation done. Patient sought consult in the OPD due to complain of increased tearing of left eye with blurring of vision.

    BIRTH HISTORY: Full term baby NSD born to 21 years old mother G1P0 at health centre delivered by midwife. No fetomaternal complications. MATERNAL HISTORY: + antenatal checkup in health centre Hx of fever during 1st trimester due to UTI-No meds taken.

    PAST MEDICAL HISTORY: Unremarkable.


    FAMILY HISTORY: + Strabismus, Both parents.

    OCULAR PHYSICAL EXAMINATIONS: VA: OD-20/50 NI OS-20/63 20/50

    FUNDUS: OD: + ROR, CM, DDB, CDR 0.3, AVR2:3,-H/E OS: + ROR, CM, DDB, CDR 0.3, AVR2:3,-H/E MOTOR: Krimsky test: Right eye fixating-35left hypertropia CR: OD-+3.50 -1.00 X 180 OS-+4.00 -0.50 X 180 Other features: Shallowing of nasolabial fold left + bells reflex Cannot smile properly Tongue deviated to left
