COURSE INFORMATION SHEET MRS.

LEWIS PARENTS AND STUDENTS--PLEASE READ THIS VERY CAREFULLY, SIGN, AND RETURN TOMORROW! WELCOME TO SCIENCE CLASS! The following information will be helpful to you throughout the year. PLEASE KEEP A COPY OF THIS SHEET IN YOUR NOTEBOOK FOR FURTHER REFERENCE! (You will be rewarded if you keep it ALL year!) MATERIALS YOU WILL NEED FOR THIS CLASS: -3 ring binder with pockets for study guides, other notes, etc. -Wide-ruled loose-leaf paper in binder -pencil -black or blue pen -Earth Science book BRING ALL THESE MATERIALS TO CLASS EVERY DAY. IF YOU NEED TO GET SOMETHING OUT OF YOUR LOCKER AFTER CLASS HAS BEGUN OR YOU NEED TO BORROW MATERIALS FROM SOMEONE ELSE, YOU WILL HAVE TO USE ONE OF YOUR FOUR PASSES (SEE BELOW). YOU HAVE UNTIL TUESDAY, AUGUST 27th TO HAVE ALL OF YOUR SUPPLIES!!! Coming prepared on Tuesday (and with this form signed) will result in a 100% or 0% homework grade, so start off on a good note! PASSES: You will have four passes (5 points each) per quarter to use for the bathroom or to borrow materials (pen, paper) if you need it. If you do not use them, you will receive five points per pass (20 points total) to use wherever you wish on your grades for the nine weeks. I use the total points system, so this could bring up a test grade or help on a missed homework assignment. If you come unprepared to class more than four times there will be consequences. GRADING: - I use the total points system. You will know ahead of time how many points an assignment is worth, and each paper I return to you will have the number of points you earned out of the number of points possible on it (ex. 43/50). If you want to know a percentage grade on any assignment, divide the larger number INTO the smaller number (ex: 43/50 is an 86%). All major tests will be worth 100 points. -I will be checking your study guides/other homework for completion & sometimes accuracy. Make sure you don't forget your homework because it's an all or nothing grade--No partial credit for late homework. MAKE-UP WORK -IT IS THE STUDENTS RESPONSIBILITY TO MAKE UP WORK WITHIN THREE DAYS OF RETURNING FROM AN EXCUSED ABSENCE. YOU ARE RESPONSIBLE FOR FINDING OUT WHAT YOU MISSED WHILE YOU WERE OUT. Please come and see me during my planning period or after school to ask about missed work if you can. I do not need to use class time to do this. -MAKE UPS WITH MRS. WEEKS- You may need to make up tests in the office. YOU MUST REMEMBER TO GO TO OFFICE AND MAKE UP YOUR ASSIGNMENTS! I will not continuously remind you. I will post who needs to turn in make-up work on the website. If you forget to make it up, it will be a zero. -SPECIAL ASSIGNMENTS: Must be turned in on time or you will lose 10% of your grade each day its late.

DISCIPLINE: My Rules: 1. Be on time for class with all of your materials. Three tardies will result in an absence. 2. Dont eat, drink (other than water), or chew gum in my classroom. 3. Dont disrupt my teaching or another students learning. 5. RESPECT SCHOOL PROPERTY, YOURSELF, AND OTHERS. The high school will be using a demerit system this year. We will take note of any broken rules and you will accumulate demerits. At a certain number, you will have to come to Saturday school & you will have to pay the teacher $10 for his or her time. ACADEMIC HONESTY: Academic dishonesty will result in referral to the administration and a zero on the assignment for you and any accomplice who may have helped you (willingly, of course). Academic dishonesty is defined as (but not limited to) sharing homework assignments, talking or passing notes during a test, copying another students work, using unauthorized materials or equipment during a test, or submitting someone elses work as your own.
