PSYCHOSOCIAL REHABILITATION CLINICAL REFLECTIVE JOURNAL Monday Aug 19th 2013 I had quite a busy day today. In the morning I administered one of the clients eye

medications ahead of his cataract surgery scheduled for Tuesday Aug 20th. Prior to this clinical I had never administered eye medication before and I was thankful to get the opportunity to do it during this rotation (5.02), (5.11). Later in the afternoon, another client self reported that he had taken his Nitroglycerine spray the previous night at 1.00 am after he felt a numbness down his left arm. The client in question has a history of angina and heart disease having received two stents last year and suffered a mild stroke just three months ago. I took the client into the treatment room to check and chart his vitals, all of which were within the normal range. (6.06), (6.09). In the afternoon another client approached me regarding the pain he was suffering on his left toe which he reported as being 8 out of 10. The client could not recall the exact details of how he sustained the injury as he obtained it along with a head injury following a seizure he suffered while sleeping. The toe was swollen with a substantial amount of pus surrounding the nail (6.07). He had being administered Polysporin PRN until he got to see a doctor which had being scheduled for Tues Aug 20th. After reviewing his file, I learnt that there were no witnesses to the reported seizure which the client had a diagnosed history. (6.06). Later in the afternoon, another client rolled his ankle while playing volleyball so I assisted the client back to his unit with the use of a wheelchair. I assisted my assigned nurse to help locate a set of crutches for the client in addition to some ice and a tenser bandage. (6.06), (6.07), (6.09). In addition the client was moved to a room on the ground floor as access to his current room was only available via a set of stairs.

Today was my final day and again I had a busy shift which I was happy with. I started the morning again with the checking of the clients glucometer readings followed by administering two clients eye drop medications. One of the clients was scheduled to go for cataract surgery, so I took the opportunity to ask him about the procedure and if he was nervous about it. (6.07). He informed me that he was not at all nervous and looked forward to getting some of his eyesight back. I also had the opportunity this morning to check another clients apical pulse as he has an irregular heartbeat for which he has being prescribed Digoxin 0.125mg QD. This was my first time to check for an irregular heartbeat which I was a little nervous about simply because his heartbeat was very irregular. (5.01), (5.04). However I completed the procedure without any concerns which was tolerated well by the client. I also had the opportunity to administer another Risperdal Consta IM to the left gluteal muscle of the client which again was tolerated well without any complaints. (5.01). The IM was witnessed by another nurse who informed me that my technique was excellent. In the late morning I assisted one of the RNs with the rollout of the doctors clinic which was pretty uneventful. I simply ensured that each client presented on time for their scheduled appointments. In the afternoon I attended another treatment team meeting for a client eligible for parole in 2014. (2.05). The client in question has a long history of drug trafficking and abuse coupled with depressive episodes leading to suicidal thoughts. I took the opportunity to ask the client about his plans to manage his suicidal thoughts if released (1.04). The fact that I felt comfortable enough to question the client helped me realize that I have become much more relaxed in this role. (1.03).
