SHEC High Potential Incident: Electric Arc Flash at High Voltage Substation
Description A Cerrejn electrician (10 years of service conducting High Voltage work) suffered burns on his left arm and left foot while he and a colleague were conducting preparation activities that were required to perform maintenance on the circuit switcher at the 110KV electrical main substation. The electrician was working with a colleague who was uninjured. The incident occurred while the electrician was extending a Telescopic Fiberglass Electrical Earth Stick and inadvertently raised the Earth Stick into the Arc Flash Zone below a 110KV bus bar, prompting an electric arc flash. The Earth Stick had the temporary bond cable attached, however it was not yet connected to the ground. Fatal Risk Control Protocol: Isolation Immediate Actions The electrician received medical attention at the mines facility and was sent to Riohacha for specialized medical attention. All electrical works at Cerrejn were stopped so as to communicate the event and reinforce preventive measures. Investigation initiated. Preliminary Findings Preliminary findings indicate that the electrician did not identify the risk of inadvertently passing the Electrical Earth Stick into the Arc Flash Zone. Preliminary Actions: Safety talks to reinforce the following: i) compliance with the Electrical golden rules, ii) risk identification for electrical works, iii) application of critical task procedures.
