8.1 Service Decision Checklist Reflection Date: October 22, 2013 This is an obvious area of concern for me as a service coordinator. I am thankful for this checklist to guide me toward what I am and am not currently doing. In this particular eligibility meeting, number 4 and 7 were done by Phyllis. 1. +/- As we were writing the goals, I would ask questions of the mother in order to elicit information to make the outcomes more functional; however, I did not always get the responses I was looking for. I need to learn to pose questions in a variety of ways in order to elicit responses that aid in the functionality of the outcomes. 3. I did not do this in this particular meeting as I was not trained to do this. I do, however, understand the importance of doing this and once I begin implementing the RBI on a regular basis, this will be done then. 4. & 7. Phyllis ensured these were done at this particular meeting. 5. +/- I did not do this for each outcome identified, rather after creating all the outcomes, I did this by overall service. Date: November 4, 2013 I felt I improved on numbers 4 and 7. I would like to work on ensuring the outcomes are functional to a greater extent. This is an area I think I do fine with, but could do better with. Typically I do not have the families prioritize the outcomes as would be done in the RBI; however, we do informally discuss which are most important to the family.

Date: November 19, 2013 This is a checklist that I feel wonderful about. I have completed this checklist three times now, and this is the exact reason I wanted to do this as my second activity. I took a baseline, and now I am working toward bettering myself as a service coordinator and meeting facilitator. I must admit, I felt deflated after scoring a 17% on the first round of this particular checklist, and only a bit less deflated with the second score of 33%. However, prior to my meeting today, I looked at the checklist, and paperwork I needed to complete and made myself a list of thing to remember to discuss at the meeting, which clearly worked. Today I feel happy with the way in which I handled this eligibility meeting. I was clear and articulate and remembered to discuss many things I sometimes forget. 3. I do not, and likely will not, have the parents prioritize their outcomes until I being doing the RBIs on a routine basis with every family.

9.1 Transdisciplinary Service Delivery Checklist Reflection Date: October 22, 2013 Parts of this checklist are completed at a couple different times. Numbers six and nine are completed at our Early Intervention team meeting the week before the eligibility meeting using a CASEtools Primary Service Provider table. The rest take place at the eligibility meeting. I used this checklist at the same eligibility meeting as the checklist above. 3. +/- I explained how services work, but did not really get into how children learn. 4. +/- I attempted, but did not follow through with back up questions. My team helped me on this one at this particular meeting. 7. + This is something we leave up to the family. Generally, the PSP visits are once a week; however, if the family feels they only need the PSP to visit once every two weeks, I will write that into the plan. 8. The goals written for this child were based around one area of concern. I look forward in the future to implementing the RBI in order to really get at other concerns the families have. 10. +/- Phyllis brought this up and I discussed it further. 11. Phyllis asked about this one.

Date: November 4, 2013 In this meeting, I improved on numbers 8 and 11, I feel as a direct result of completing this checklist previously. Date: November 19, 2013 As with the 8.1 Service Decision Checklist, I am feeling better about this checklist every time I complete it. Although, I did not score quite so low on this one the first two times, I was still fairly deflated. However, I did the same thing this morning with this checklist as I did with 8.1, and was able to better implement the items on the checklist. 4. +/- I do not feel comfortable giving myself a + on this item without actually having done an RBI. I do collect information on the family routine to some extent on the first visit, and discuss times of day during the creation of the outcomes; however, it seems as though this item is specifically talking about an RBI.

10.2 Support-Based Home Visiting Checklist Reflection Date: November 7, 2013 This was the first time I have been able to go to a home visit with Phyllis without an agenda of my own. I was happy to be able to see how a home visit generally goes for a PSP. I completed some of the checklist during the home visit, but most of it was completed when Phyllis and I debriefed about the visit on the way back to the office. In addition to a general conversation about the visit, we went through each item on the checklist together. 3. +/- , 14. Toward the end of the visit, the father covertly brought up an issue related to the childs sleep and waking in the middle of the night. Phyllis did not feel she had time to address that at this home visit, but will bring it up at the next visit. 15. Rather than asking or not showing at all, Phyllis said, This is what you might see while Carly is playing with [child]. 18. +/- Phyllis touched on most of the outcomes but not all. It is my feeling that sometimes there is not enough time in home visits to touch on each outcome, particularly once we start implementing 10-12 outcomes with the RBI. 21. +/- Phyllis asked about the older brother in relation to the outcomes. 29. N/A Many of the outcomes discussed today were based on mealtime routines and we were not there during mealtimes.

Date: November 12, 2013 It is interesting how the checklists varied from family to family. The two families varied quite drastically, as did the checklists. Things from the first completed 10.2 checklist that were scored a +/- or , did not necessarily receive the same score on the following home visit checklist. However, some items that were scored with a + on the first checklist scored a +/- or a on the second. There did not seem to be rhyme or reason to this, other than the fact that they were two different home visits with two different families. 14. This was actually one item that was scored the same on both. This time, Phyllis did not use these exact phrases; however, she and the mother are already working toward solving the problems that arose during this home visit. 19. I scored this item this way for this checklist; however, on the first wrote N/A because we are not implementing this practice as a site yet. Date: November 25, 2013 This checklist was completed with the same family as the checklist dated November 12. 26. & 29. These opportunities either did not come up, or they were not asked about at this home visit.

11.1 Consultation Checklist Reflection Date: November 12, 2013 This was an interesting visit to be able to observe. It did not really go how I thought it might, given the small amount of experience I have in collaborative consultation. I was not able to see the first few minutes of the session as I was speaking to the mother of the child outside the classroom. Phyllis filled me in on what was discussed prior to my coming into the classroom. In all actuality, collaborative consultation is a difficult thing to do well if the only time you and the teacher have is while the children are present. This took place in a classroom with two teachers and eight children under three-years-old. For the last quarter of the visit, they did a painting activity, which took all of the teachers energy and attention. Together we fil led out the checklist after the visit. 4. +/- They addressed some of the outcomes, the ones that were of top priority to the teachers that week. 20. +/- I did not hear this, but Phyllis let me know she touched on independence and social relationships through the initial conversation with the teachers. 21. +/- Phyllis and I discussed the specific situations in which this took place throughout the visit; we agreed it could have taken place at a few other points as well.

Reflection One of the top reasons I am excited to have implemented these checklists is for personal gain. I treated these checklists as a method to obtain a baseline on how I was doing in my new role as a service coordinator. I feel great about some of the checklists; however, there are several that I had to work hard to improve. Although I will not continue to complete all of these checklists, I will continue to work toward constantly becoming better at this role, and will continue to use these checklists as tools, and refer to them for guidance. Another reason I was excited to use the checklists was to not only look at them in terms of my own work, but also how they might fit into our agency for my regional site, as well as on the state level. There was some difficulty in implementing these checklists. I am a bit of a perfectionist, sometimes to a fault. Some of these checklists, for example 8.1 and 9.1, showed areas of significant weakness for me as a service provider. I found it difficult to assess myself in areas in need of great improvement because I like to do things well; I find it hard to admit when I am not good at something. However, without these checklists, I may have gone on conducting these meetings as I had been without acknowledgment of a need for improvement. The great thing about these checklists is I now know the exact things I need to work on from here on. I feel fortunate I was able to use these checklists as a baseline, and then continue to work with them as a method of improving my service coordination and home visiting skills throughout the semester. Something that excites me is dreaming about ways I can make an impact on CDS at both my regional site, as well as on the state level. I believe my work with these

checklists can benefit people from both. Perhaps this is how I can make a difference: aid in the process of the RBEI implementation. I feel once I know the checklists well, I can help to teach others how to make use of them. For the first half of the semester, Phyllis has been the only PSP completing them with me. However, after talking with Phyllis, and the entire Midcoast Regional CDS Part C team being introduced to three of the checklists in a team meeting in late October, I asked other team members to assist me in completing some of the checklists, when appropriate and if they are comfortable with it. This not only benefited me as a service coordinator, but it was also a great way for at least one Part C team member to begin familiarizing themselves with the checklists that may be used in the CDS system in the not too distant future. Overall, implementing these checklists benefitted me immensely. They were a great way to provide me with a large amount of information pertaining to many different areas of my role as a service provider. I am pleased I thought of this as an additional activity, and look forward to continuing to refer to them after this course is finished, and perhaps beyond that as part of a larger project. My hope is to be able to use these not only for my own benefit, but also to benefit others in the agency.
