(Downtime at zero rate: 06-06-12 = 16 hrs, 07-06-12 = 24 hrs, 08-06-12 = 8 hrs, 09-06-12 = 12 hrs,
10-06-12 = 24 hrs, 11-06-12 = 3 hrs totaling 88 hrs).
EVENT DETAILS: CUVC card not working in encoder normal mode. The client would not allow us to drill out the shoe till the CUVC
board was within 24 hrs of arriving on location nor perform the coring operation in the encoder by-pass mode until we had been
attempting to program the parameters unsuccessfully till 0100 hrs on 11-06-12. The spare CUVC board was found to be faulty. Mobilized
1 CUVC board from Pakistan & 2 CUVC boards from NOV Houston. The CUVC board arrived on the rig site after drilling the casing shoe
& making the trip for the core barrel. Installed the new CUVC board in the TDS VFD house. Attempted to program the parameters on the
new board unsuccessfully. Communicated with Dubai on resolving the programming many times. Dubai involved many techs in assisting
over the phone & by e-mails on resolving the problem. Mobilized 2 Siemens techs from Dhaka to the rig site to render assistance. The
Siemens techs were unable to come up with a solution but were very helpful on using the software, etc. The client finally gave the go
ahead for coring in by-pass mode at 0100 hrs on 11-06-12. After cutting the core & POOH till the core head was above the shoe we
continued to troubleshoot the reason the new CUVC board was showing a communication error. We finally found problems in the
parameters we had uploaded from the old board to the new board. We resolved the issue by changing the power definitions then
uploaded the parameters to the new CUVC board again with no rejections. Test ran the TDS & it performed 100% correctly with the
encoder in the normal run mode.....................................................................................................................................................................

ROOT CAUSE: Lightning strike faulted out the encoder processing portion of the CUVC board. The spare CUVC board in the rig stores
was faulty. Personnel on the rig were unfamiliar with the process of programming the new CUVC board. ....................................................

SUGGESTED CORRECTIVE ACTIONS: 1 ea ET on the rig now with no B2B. Hire another ET as B2B to ensure an ET is on the rig 24x7.

DESCRIPTION OF FUTURE PREVENTATIVE MEASURES: The new spare CUVC board that we have in the rig stores is preprogrammed for the TDS. There is an additional CUVC un-programmed board in the rig stores for use on other equipment. The
programming instructions are in a file on the electricians desktop. ..............................................................................................................
