Abstract: [This abstract is based on the authors' abstract.]In a effort to coordinate conditions
related to patient discharge, Japanese hospitals have found it necessary to determine the next
facility to which the patient will be transferred, based on the patient's needs and those of his or
her family. A model is proposed that enables social workers to conduct discharge coordination
efficiently and correctly. The model is presented as three outputs: a list of canvas items to be
considered during discharge, a flowchart of the discharge coordination process, and concrete
components of the flowchart. The efficiency of the model is demonstrated in application to actual
cases.

Abstract: The medication error rate at Illinois' Alton Memorial Hospital was low when compared to
national statistics, but the hospital knew cost savings would be significant if this statistic was
lowered even further. A multidisciplinary team sought the help of a Black Belt to help the team
use the Six Sigma define, measure, analyze, improve, and control (DMAIC) methodology to
reduce the defect rate of the current process and to develop a standardized process. While
changes improved the medication order entry process, the issue of the multiple ways the staff
process medication orders remained. The team then decided to use a design for Six Sigma
methodology, quality function deployment, to link the needs of the customers to the development
of the process functions. Audits revealed the percentage of order entry errors improved by 90
percent. Additionally, the new process is simpler, works on all units, and is more accurate.

Abstract: [This abstract is based on the authors' abstract.]In 2004 ProMedica Health System's
ProMedica Laboratories at the Toledo Hospital introduced lean manufacturing techniques as part
of its continuous performance improvement efforts. The focus of these efforts was on identifying,
labeling, and eliminating waste. A quality expert was retained to train core team members from
multiple laboratory departments about lean concepts. This grassroots approach ensures early
staff by-in, transfers knowledge to a broader base of in-house staff, and minimizes cost to the
organization. ProMedica Labs has not added any additional staff for Lean, but is reinvesting
productivity gains in additional release time for staff to engage in training and projects. Upon
completion of three projects, individual team members are qualified to facilitate teams and train
others to apply the methods to their work areas.

Abstract: [This abstract is based on the authors' abstract.]A study was undertaken by the Oregon
Department of Human Services to determine if sociotechnical probabilistic risk assessment (ST-
PRA) can create risk models identifying combinations of medical delivery system and behavioral
elements resulting in medical error in long term care facilities. Multidisciplinary teams created the
models with input from pharmacists, community physicians, and state surveyors. Results showed
that prescribing and administration errors are the dominant risks. In addition, multiple single
failure path errors were identified. State agencies, long term care providers, pharmacies, and
medical providers are collaborating on strategies to address the risks identified in these models.
Abstract: In 2004, North Shore University Hospital (NSUH) used Six Sigma to reduce bed
assignment delay turnaround time that made it difficult to balance capacity needs. A capstone
project revealed incorrect use of the hospital's bed tracking system. The goal was to shift the
mean turnaround time and reduce the standard deviation. The team discovered both a
communication and technical failure in two key steps in the process that caused great delay.
Once the project was turned over to the nursing management team, the process went from a
turnaround time of 226 minutes to 90 minutes, and the Six Sigma score went from 1 sigma to 2.3
sigma at the end of the six-month project. The project has served as a pilot for the rest of the

Abstract: The principles of lean manufacturing are as applicable to healthcare as they are to the
automobile industry. However, unlike manufacturing, healthcare management structures are not
usually hierarchical, and hospitals generally are not-for-profit. Value adding for patients is
different than for customers in manufacturing because the goal of healthcare is prevention and
cure. The 18 building blocks of lean healthcare can eliminate waste and prevent the propagation
of errors. Two 5S projects show how staff members were benefited and lean efforts were
encouraged throughout the organization when results were achieved quickly.
