PROGRAM DESCRIPTION (INCLUDING TARGET POPULATION, LOCATION, DATES/TIMES)

The Food and Fitness for Families program will be a six-week program for families to teach them better
nutrition habits and help increase their physical activity levels. The initial program will be run through
Lake Forest Church in Huntersville. It will include nutrition/cooking classes held in the lobby and kitchen
of the church for both parents and children every Wednesday evening from 6:30 to 7:30 beginning
October 3, 2012, to November 7, 2012. The families will be taught some easy healthy recipes, which will
be served at the class. The physical activity events will be held on Saturdays from 10:00 to 11:00
beginning October 6, 2012, to November 10, 2012. These will be held at Jetton Park in Cornelius. Each
week will feature different family-oriented physical activities utilizing the walking/biking trails that can
accommodate many different fitness levels, the tennis courts, basketball courts, and water activities.
There will be incentive prizes awarded for different competitions.

The program will be advertised through the church website, weekly bulletins and flyers, and promoted
through word of mouth throughout the different groups within the church. At the end of the program,
families will be surveyed for satisfaction with the program, as well as utilization of recipes and increase in
their activilty levels.

GOALS:
1. To educate families about better nutrition choices and how to increase physical activity levels.

OBJECTIVES
1. By the end of the nutrition classes, at least 80% of participants will have learned one new healthy
recipe to include in their family meals.
2. By the end of the physical activity events, 60% of families will have followed through with
increasing the physical activity levels of their family.



TASKS AND TIMELINE (INCLUDING MARKETING TASKS AND EVALUATION TASK/DATES)
8/2: Meet with church officials and coordinate
plans with them for the event, i.e. meeting times,
advertising plans, necessary consents, recruiting
volunteers, funding available. Submit advertising
for church bulletin and webpage.

8/3: Contact Jetton Park to make reservations for
any areas that require them. Print out flyers to be
handed out at weekly services, consents, and
surveys.

8/4: Hand out flyers at services (ad will be in
bulletin every week).

8/15: Planning meeting for volunteers at church.

8/18: Order T-shirts and other incentive items for
physical activity events.

9/1: Second planning meeting to discuss recipes
and physical activity exercises, assign volunteers
to areas of interest..




9/2: Conduct registration sign up during church
services, along with ongoing registration through
advertising.

9/15: Final planning meeting---determine supplies
needed for recipes and activities.

10/3: Registration and pre-participation surveys.

10/3, 10/10, 10/17, 10/24, 10/31, 11/7: Nutrition
classes at church from 6:30 to 7:30 pm.

10/6, 10/13, 10/20, 10/27, 11/3, 11/10: Exercise
activities at Jetton Park from 10:00 to 11:00 am.

11/7 and 11/10: Participant satisfaction and post-
participation surveys.

11/17: Summative evaluation and meeting with
church officials to discuss further program plans.

EVALUATION PLAN (INCLUDES DATES; USE PROFESSIONAL TERMS)

A pre-participation survey will be done at registration to provide base levels of physical activity, nutrition
habits and knowledge. At the conclusion of the program, participants will complete a program satisfaction
survey, as well as an assessment of any increase in family physical activity levels and recipes learned
and incorporated into family meals. Volunteers will also provide an evaluation of program and any
feedback of ways to improve it.

A summative evaluation report will be completed and discussed with church officials to make decisions
on expanding or continuing program and any changes that would be beneficial.
