Dear Parents,

This week, your child has been studying cultural changes in America during the 1920s. One o these
changes in!ol!ed the creation o "a## music, which took the nation by storm during this time $eriod. %
am e&cited to announce that our class has the o$$ortunity to !isit the Alabama Jazz Hall of Fame,
located in 'irmingham, on May 15, 2014(
All students in the class are in!ited to accom$any us on this tri$. )e will de$art rom school at 8:15
am, and we will return by 11:30 am. Our tri$ will include a guided tour o the *all o +ame, a !ideo
$resentation, and a sack lunch $icnic on the $layground once we return to school. ,tudents will also
ha!e a snack break around 10-00 during the !ideo $ortion o the tour, and snacks will be $ro!ided by
the school. .our child is welcome to $ack his/her own lunch or our $icnic on the $layground, or he/she
may $ick u$ a sack lunch rom the caeteria once we return to school.
The cost o this tri$ is $10, which will co!er bus ees and admission to the *all o +ame. Any e&cess
unds that are collected will be added to the ield tri$ budget or si&th grade. % would also like to in!ite
three $arent cha$erones to accom$any us at the same $rice o 010. Please email me at

The teacher will carry a bag o any student medications that need to be
administered during the tri$, along with a list o students whose medications
will be administered and at what times. Parent cha$erones will be gi!en a co$y
o this list.
The teacher will bring a irst aid kit with basic su$$lies- band=aids, >eos$orin,
gau#e, ta$e, tissues, and calamine lotion. Any minor medical issue should be
re$orted to the teacher, and the irst aid kit may be used.
,erious medical emergencies should also be re$orted to the teacher, as well as
the sta o the *all o +ame. Cmergency ser!ices can then be called to the
acility.
All medical issues, regardless o se!erity, will be re$orted to the student4s
$arent. The teacher and all $arent cha$erones will carry a list o A?? $arent
contact inormation.

s$eciic medical concerns. 6ha$erones will also ha!e a list o all $arents4

any $roblems to the teacher immediately.

All students will wear their blue school t=shirts on the day o the ield tri$ or
easy identiication.
,tudents must stay with their $arent cha$erones at all times. Any $roblems
should be re$orted to the cha$erone, who can then re$ort it to the teacher.

will sit near his/her grou$ o students.

will be waiting or us when we arri!e so that we can take them to the
$layground or a $icnic.

accounts. A list o these students will be sent to the caeteria sta the day
beore the tri$.
,tudents may $ack a lunch rom home i they do not wish to $urchase a sack
