1. OBJECTIVE This document defined the PETRONAS 10 Zero Tolerance Rules adopted from MLNGs original 5 Safe Life Zero Tolerance Rules and PETRONAS 10 Zero Tolerance Rules. These Zero Tolerance Rules are defined to ensure that all activities are carried out in a safe manner and any non-compliance is not tolerated.
The PETRONAS 10 Zero Tolerance Rules are to raise HSE standard to achieve HSE Excellence and making HSE Excellence happens to minimize HSE incidents, and prevent people from getting harm.
2. SCOPE The PETRONAS 10 Zero Tolerance Rules are the minimum mandatory Zero Tolerance requirements that each and every employee and contractor working in and visitor to PETRONAS LNG Complex (PLC) shall comply.
3. ACCOUNTABILITY/RESPONSIBILITY HSE shall be accountable and responsible for developing and maintaining this document.
All MLNG managers and supervisors are held accountable to communicate these Zero Tolerance Rules to all their staff, contractor staff and visitors and ensure they understand the need to comply with these Rules and the consequences of breaking those Rules.
Full commitment and cooperation from all staff, contractor staff and visitors at site is required to make MLNG a safer work place. All are to share a common objective and, that is, each day everyone comes to work happily and at the end of each day everyone reaches home safely.
4. DEFINITION Term Definition Zero Tolerance Zero Tolerance, a principle to ensure all activities are carried out in a safe manner and any non-compliance is not tolerated. 5. PREREQUISITES All employees and contractors shall have been briefed on PETRONAS 10 Zero Tolerance Rules and Consequence Management for non-compliance by MLNG managers and supervisors.

6. PETRONAS 10 Zero Tolerance Rules IN BRIEF

7. MAIN CONTENT
7.1 INTRODUCTION PETRONAS 10 Zero Tolerance Rules are based on analysis of incident trends in the PETRONAS Group and aim to ensure that all high risk activities are carried out safely.
Zero Tolerance, a principle to ensure that all activities are carried out in a safe manner and any non-compliance is not tolerated.
MLNG employees, contractors and visitors shall comply with 10 Zero Tolerance Rules at all times.
This, however, does not disregard other safety rules and regulations that are imposed by MLNG
These rules shall not replace or invalidate MLNG other business, operational and safety rules which are already enforced at our workplace.
All other safety rules and regulations shall be abided by at all times.
NOTE In MLNG the words Zero Tolerance shall not be condensed to ZeTo. This is to demonstrate the leadership and commitment of MLNG management towards securing the safety, health and welfare of workers and the environment as well as to communicate with emphasis the seriousness of non-compliance to all The PETRONAS 10 Zero Tolerance Rules which are the minimum mandatory Zero Tolerance requirements that each and every employee and contractor working in and visitor to PETRONAS LNG Complex (PLC) shall comply.

7.2 TEN (10) SAFE LIFE Zero Tolerance RULES
7.2.1. Permit-To-Work
Work with a valid work permit (PTW) and comply with its requirements
You shall:
Commit to obtaining a valid permit/certificate and understand the requirements stipulated on it; Communicate the requirements to your work team members; Comply with the requirements to eliminate/reduce the risks; Consult when in doubt.

Been briefed by your supervisor on the safety requirements of the job? Ensured compliance to all requirements of the PTW? Been trained and are competent to carry out the job safely? Identified the hazard and assessed the risks? Put in place appropriate control and recovery measures? Communicated the permit and certificate requirements to your team members? Displayed the permit and certificate at the place of work? Obtained the right tools and equipment and ensured they are in good condition? Obtained the correct PPE for the job? Been briefed on the emergency procedure?

7.2.2. Energy Isolation
Verify energy isolation before starting work
You shall:
Commit to verify the required lock out/tag out before work begins and use the specified life protecting equipment to work on the right equipment ; Communicate and confirm on site that the equipment is correct and that positive isolations are in place; Comply with the requirements from approving authority instructions and confirm that it is safe to start work; Consult with approving authority and stop work when you sense that something is not right

Checked that the PTW and relevant certificates such as physical isolation certificate, electrical isolation certificate and radiation certificate have been approved? Checked that the right piece of equipment or machinery has been isolated, locked and tagged? Secured any movable equipment or machinery which you are working on? Confirmed that all stored energy has been safely released? Secured an individual or department lock at the relevant lockout box? Ensured that all guards and safety systems are put back when work is completed?

7.2.3. Overriding Of Safety Critical Equipment
Obtain authorization before overriding or disabling safety critical equipment
You shall:
Commit to obtain authorization before overriding or disabling safety critical equipment & do a risk assessment; Communicate the risk associated; Comply with the recommended remedial actions; Consult and continually monitor the process parameters during overriding duration.

7.2.4. Confined Space
Obtain authorization before entering a confined space
You shall:
Commit to obtain authorization before entering a confined space and do a risk assessment; Communicate the risk associated; Comply with the requirements stipulated in PTW and Confined Space Certificate; Consult and continually monitor the conditions of the confined space.

7.2.5. Working At Height
Protect yourself against a fall when working at height above 2 metres
You shall:
Commit to fall prevention and protection by
checking for green scaffold inspection tag ready for use; using body harness with double lanyard; barricade work area at ground level with No Entry signs posted
Communicate to all workers on wearing full body harness and 100% tie-off; Comply with securing tools and equipment; Consult with Work Supervisor on donning of body harness and anchoring points.

Been briefed by your supervisor on the safety requirements of the job? Ensured that the platform has proper guard rail and access ladder? Used only approved scaffolding with valid inspection tag? Secured your tools and equipment to prevent them from falling from height? Ensured three points of contact is maintained at all times when using the ladder? Worn a full body harness with double lanyards fastened to a suitable anchor point?
NOTE When working above two metres
At the edge of a platform or other locations where there is risk of falling from height OR During installation of a scaffolding OR While working on top of a ladder, if the job requires it

7.2.6. Personal Protective Equipment
Wear and use personal protective equipment (PPE) at all times
You shall:
Commit to wear and use correct PPE at all times; Communicate the PPE requirements to your work team members during the daily tool box talks; Comply with the PPE requirements posted at site and stipulated in permit-to-work; Consult PPE instruction and check your PPE each and every time before use.

Checked the operational condition of the nearest eye wash, emergency shower and spill cleaning kit?

7.2.7. Excavation
Obtain authorization before excavation or entering a trench
You shall:
Commit to obtain authorization before excavation or entering a trench; Communicate the risk associated; Comply with the requirements stipulated in PTW and Excavation Certificate; Consult and continually monitor the conditions of the excavation or trench.

Been briefed by your supervisor on the safety requirements of the job? Checked for buried services and other hazards? Checked that the issued PTW is complete with Excavation Certificate? Ensured that emergency plans and procedures have been put in place? Excavated manually using spade with wooden handles to expose buried services before using mechanical equipment? Ensured a correct shoring system is put in place with appropriate underground protection measures? Erected rigid signs and barriers around the excavation? Provided safe access and egress to excavation trenches? Ensured confined space entry procedure is applied for excavation of more than 1.5 metres deep?

7.2.8. Lifting Operation
Do not position yourself under a suspended load
You shall:
Commit to preparing a lifting plan and obtaining a lifting certificate; Communicate the plan especially on the risk associated; Comply with the requirements stipulated in PTW, Lifting Certificate and checklist; Consult and closely monitor the lifting operations.

Been briefed by your supervisor on the safety requirements of the job? Checked that all lifting equipment have been tested and certified by authorities? Obtained a PTW and Lifting Certificate? Ensured that the operator, rigger and signal man are competent and certified? Ensured a lifting plan has been prepared and the lift is assessed by a competent person for critical lifting? Checked that the load is within the safe working load of the lifting equipment? Checked that the tagline is attached to the load? Barricaded the area?

7.2.9. Ignition Sources
Do not smoke outside of designated areas or bring in potential ignition sources into manufacturing areas without authorization
You shall:
Commit to smoke in designated areas and obtain authorization before bringing in any potential ignition source; Communicate the risk associated; Comply with MLNGs requirements on ignition sources; Consult your supervisor on whether your equipment or tool is a potential ignition source and continually monitor its use.

Been briefed by your supervisor on the safety requirements of the job? Rectified or eliminated leaks immediately? Ensured hot work permit is obtained and gas tests done before carrying out hot work? Obtained hot work permit before using engine-driven equipment, vehicles and any other potential sources in process areas?

7.2.10. Driving
Do not use your mobile phone/walkie-talkie while driving, follow the speed limit and use your seat belt
You shall:
Commit to making your car a No Phone and Buckle Up zone; Communicate with passengers to fasten seat belt and do not drive until all passengers are securely fastened; Comply with the MLNG traffic rules and regulations; Consult and continually monitor the conditions of your vehicles.
Have you
Been authorized to operate the vehicle and have a valid driving license? Completed the specified daily checks and ensured the vehicle can be operated safely? Checked that the seat belts are available and worn by all occupants? Secured all materials and equipment carried on the vehicle? Adhered to all traffic regulations? Ensured that the vehicle is used for its purpose? Ensured that your vehicles engine is not left running unattended?

7.3 CONSEQUENCE MANAGEMENT MLNG employees and contractors personnel working in PETRONAS LNG Complex shall comply with Zero Tolerance Rules failing which shall constitute serious misconduct and may result in disciplinary being taken.
Contractors failure to comply with Zero Tolerance Rules can result in termination of the contract and/or other appropriate action being taken.
Anyone found breaking these rules will be investigated thoroughly and if the violator is aware of these rules, the failure to comply with MLNG PETRONAS 10 Zero Tolerance Rules shall mean that he/she has chosen not to work with MLNG and shall result in following disciplinary actions:
For staff, up to and including termination of employment. For contractors, removal from site and disqualification from future MLNG work.
