Dear Madame,

Greetings!

This letter is to formally inform your good office regarding my intention to be

an assessor for Health Care Services NC II.

My qualifications to become an assessor are as follows: I am a graduate of

Bachelor of Science in Nursing.

I took and passed Health Care Services NC II trade test last October 4, 2008
with Certificate No. 08030302004986 and I attended TQAQ for trainers just
last Nov. 12 – 16, 2007 .

At present, I am a regular employee at the Divine Colleges of Malolos City

and a Caregiver Instructor as well.

I am looking forward for your response granting me to become an assessor

for Health Care Services NC II.
