1.  Internet application forms without fees (Rs.10000/­ & Rs. 150/­) will be rejected and 
all fields in the form must be filled by the applicant. 
2.  Attach attested copies of educational qualifications. 
3.  Following fee is to be deposited along with the internet application form: 
(a) Application form fee of Rs. 150/­ (only for downloaded forms) and The 
initial admission fee of Rs.10,000/­
