3. Who will score the assessment, if scored?
a. This is guided practice, it will not be scored. Students will have an opportunity to try the
problems on their own and then students and teachers will work through the problems
together as a class. Students can make notes or corrections on their form with green pen.
4. Describe the info (data) that will be collected.
a. Individual guided practice worksheets will be collected. Teacher will make observations of
students individual work, indicated in pencil, as well as areas where the student determined
that they needed to make corrections, indicated in green pen.
5. What will the teacher do with the data? How will feedback be given? Will the scores be used for
grading? If so, how?
a. Work will be collected from each student at the conclusion of the lesson. They will be reviewed
by the teacher, looking at the students work for errors in the process. The ticket out the door
result will be on the board at the beginning of the next lesson. Students will be able to
compare their answer to the correct answer. It will not be graded.
6. What is the plan for students to use the information collected?
a. Students will be able to also check their understanding of the concept by comparing their work
to the sample answer. Students will be able to compare what they did to solve the problem to
how the correct answer was reached.

An X will be placed in each column with a correct answer. The last column serves as an area to record
information about errors that the student made while calculating the answer that may be helpful with
correcting the error with the student.
8. Describe intentional, possible interventions for low scoring students (those who dont get it) and
enrichments for high scoring students (those who get it the first time).
a. This information will be used to determine areas for instruction during the next lesson.
Students will work in groups based on the results from the assessment. Students could be
placed in one of three groups: students that understood both concepts and will be doing
enrichment activities, students that will be working on creating ratios, students working on
simplifying fractions, and students that need to work on both areas. For students that need
more help they will use manipulatives and instructional videos to increase understanding.
Students that understood the concepts will work on enrichment activities. They will be given
more difficult ratios to construct and simplify as well as problems in story problem format.
This will encourage students to think about the information that is necessary and not
necessary and to correctly construct the ratios based on the given information. As students
are working the teacher will rotate through the groups to check for understanding and provide
additional instruction and assistance.
