Alert on Quality issues

Subject: Alignment and erection issues of Non separable phase busduct at PGPL site.
Purpose of this alert:
This alert is meant to reduce the occurrence of below described wrong practices at any of TPL
sites and steps to be taken to foresee and prevent such practices.
1. Description of Quality issues:
It has been noted at Aishwariya well pads that Electricians/Technicians are tampering weather
proof/flameproof cable glands by cutting/removing compression bushes in double compression type
cable glands. This is done to make their work faster under project pressure. Below mentioned is the
direct and indirect impact of this wrong practice.
1.1.Direct impact:
a) The tampering leads to tampering of Ingress protection properties and flame proof properties of the
particular cable gland.
b) Tampered cable gland thus becomes useless and contributes to waste of material and overhead cost
to project.
1.2.Indirect impact:
a) Customer gets unsatisfied due to wrong practice being followed and affects our quality assurance
commitments.
b) Replacement of the tampered cable glands and reglanding results in delay in progress and adds to
the rework and material cost.
c) If material required is not available for large quantity replacement, it eventually leads to material in
sufficiency and again delay the progress.
2. Root cause:
a) Mis-match in cable outer diameter and the procured gland size (Engineering or Procurement issue).
b) Reluctance (Deliberately/unknowingly) in highlighting the issue to concerned supervisor or Site
Engineer
3. Correction done:
1. Replaced all rubber bushing of all tampered cable glands.
4. Preventive action done:
a) Educated all site supervisors and technicians/Electricians about the seriousness of this wrong practice.
b) Strictly instructed the concerned supervisors and technicians to inform their in charge about any kind
of mis matches before they provide their own easy solutions.
C) Advised Site Engineers to check in advance the size mis-matches between the materials available.
