Outside Agencies and Post-Graduation Support


From childhood through puberty and adolescence to adulthood, transitions are
inevitable and remain as a fact of life. For individuals with disabilities, their experiences
of transitions are quite different from yours and mine. Often faced with additional
challenges and barriers, transitions for individuals with disabilities can be and
overwhelming experience- this is especially true for preparing and planning for life after
k-12. Since 2004, schools across the nation are now required to include transition
planning into students Individualized Education Plan, which addresses life after high
school graduation. From vocational education to adult services and community
participation, transitional services are unique to each individual and are based on
individual needs, strengths and preferences. The following are just a few of the services
provided throughout La Crosse County.

By adhering to the Americans with Disabilities ACT and to State and Federal Fair
Housing Law, La Crosse County Housing provides affordable housing for seniors,
disabled and low-income families. For individuals to be eligible they must meet specific
income requirements and must complete an application (with management) to be
considered for housing.

Riverfront Inc. is a non-profit organization that serves to support individuals with
disabilities. At Riverfront Inc. they strive to be the sources of facilitating individuals
ability to live, learn, and work. From employment services to housing options and
recreational activities, Riverfront provides personalized services and programs that are
unique to each individual.

Funded through the United States Department of Labor, the Wisconsin Department of
Workforce Development and the Western Wisconsin Workforce, Workforce Connections
provides opportunities for individuals who have experienced barriers in their ability to
obtain and maintain meaningful employment. Contracted with the Division of
Vocational Rehabilitation, Connections to Independence (CTI) is focused on supporting
individuals with disabilities in discovering employment options and services.
