Goal Statement: To continue my education as I work in the acute care setting, allowing me to provide excellent patient care with great competence
and compassion.

Elements
Objective(s): Describe what
you want to accomplish in
your first two years in
practice.

Plans/Strategies: Describe
the actions that you will take
to achieve your objective.

graduating from
Kapiolani Community
College.
2. Apply and be accepted to
the new grad program at
Hawaii Pacific Health in
either the inpatient or
surgical program
subsequent to
graduation.
Upon graduation I will begin
work as a Clinical Assistant at
Straub Hospital on the Resource
Team. With this job I will train
and work with experienced
Clinical Assistants and
Registered Nurses to learn
patient care in the acute setting.
Also working on the Resource
Team, I will have the privilege of
caring for patients on 4 units
plus the Surgical Charges
Department. Being able to work
with patients in different units will
give me a broader range of
knowledge. I will care for
patients in the regular Medical
Surgical Units, the Critical Care
Intermediate Unit-Telemetry, and
the Burn Care Unit. I am excited
to cross train across the units to
help me become a more
qualified competent nurse.

During the 2014 summer, I had

the opportunity to intern at Straub
Hospital. My mentor is the
Manager of Hospital Operations.
I have had the invaluable
opportunity to see the inner
workings of a hospital. I have
shadowed my mentor to manager
meetings, leadership meetings,
and the RN skills fair. I continue
to work at Straub as an intern,
and currently am assisting the
Stroke Team, readying Straub
Hospital to be certified as a
Primary Stroke Center by The
Joint Commission. I am also
working with my manager on the
National Quality Surgical
Infection Program to decrease
the surgical site infection rate at
Straub Hospital. The internship
has and continues to be a
learning opportunity that enables

is a lesson in leadership that has
taken me beyond the classroom.

compassion.
Reliable: I am
dependable, and will
follow through on
assignments I am
responsible for.
Hard worker: I strive to
take initiative and
perform tasks with
excellence, taking
responsibility for my
actions.
I am up to date on the
required skill
competencies.

Areas targeted for growth:

From your own self
assessment, describe what
areas in yourself that you
would like to strengthen.

1. The primary area of growth I

would like to strengthen is my
knowledge of pharmacology.
Two reasons for my focus of
pharmacology is medication is a
considerable portion of nursing
care, and medication error is an
event I am focused on avoiding
during my career. I will do this
by:

Dedicated: I aim to
exceed expectations, and
willingly take on additional
tasks. I will not say,
Thats not my job.
Honest: I keep
commitments. I regularly
reflect and evaluate my
actions. I appreciate
feedback, both positive
and constructive criticism.
Positive Attitude: I believe
I am easy to get along
with, willing to help others,
and willing to learn new
things.

Life long learner: I enjoy

learning new things, and
applying them to become
better. I look for classes or
seminars, and attend them
when my schedule allows.

Detailed oriented: I pay

attention and take pride in
my work.

o Minor in speech

Improve my speaking
skills to become an
effective public speaker
and communicator.

Become a reader.

Memorizing drug brand

Reviewing drug classes
Reviewing side effects to

Knowing patient
education topics that I
can teach my patients
each time I administer
medications.

2. The second area of growth I

would like to strengthen is IV
starts. This is a skill I desire to
execute with excellence, as I
have observed it brings much
anxiety to patients. Ideally I
would like to successfully
administer IV starts on the first
attempt. I plan to look into
classes or research the topic
after graduation.
