AFFIDAVIT OF ADMISSION OF PATERNITY

I, NIKKO S. GIRON, of legal age, Filipino citizen and a resident of
Tacloban City, after having been sworn to an oath in accordance with
law, hereby depose and state: THAT--1. I am the biological father of a girl registered in the Register of
Births on page 9999 of Book No. 999 of the Office of the Civil Registrar
of Tacloban City, under the name of NIKKA MAE S. VERMUG, born on 15
January 2009 at Tacloban City;
2. There was no entry of the father name in the aforesaid
Certificate of Live Birth, and that said birth was registered but we were
not yet legally married with the mother of the girl until now;
3. It is only in the Certificate of Baptism that the names of both
parents of Nikka Mae S. Vermug are indicated, viz: the undersigned as
father and Robilyn S. Vermug as the mother;
4. In order to develop the love of the herein child towards me as
her father, it is now my great desire to admit her as being my
legitimate child in order that her record of birth is properly annotated
and my name be recorded by virtue of this Affidavit of Admission of
Paternity;
5. I am executing this affidavit to attest to the truth of the
foregoing facts for whatever legal purpose/s this may serve.
