ThemissionofLloydchellsLittleLambsChildCareistoprovidehighqualitycarethat
includesadevelopmentallyappropriatepractices;knowledgeableandwelltrained
educator.Thisincludescomprehensiveservicesthatsupportchildrenshealth,safety,
nutrition,andsocialwellbeing.
ENROLLMENTPROCEDUREANDFEES
PriortoyourchildsstartdatethefollowingmustbereturnedtoLloydchellsLittle
LambsChildCare:AcompletedEmergencyFormwithcurrentimmunizationrecords
andfirstweekcharges.
Licensedchildcarewillbeginwitha2weekperiodtrialperiod.Thefollowingterms
andconditionswillbemetforpermanentchildcareplacement:
Parentorprovidermayterminatechildcareservicesduringthetrialperiodwitha48
hournotice.Thereisnoregistrationfee.
Thestartingdateofchildcareserviceswillbe

Thechargeforthecareofthechildis:

PaymentsandfeesaredueinadvanceonaweeklybasistheFridaybeforetheweek
beginsorthelastscheduleddayofattendancefortheweek.Paymentobligationisbased
onthehoursagreedtousechildcare,notonactualattendance.Thereisnochangeinfee
duetoyourchildsabsences.IfyourchildisabsentorIamclosedontheFridaybefore
theweekbeginsorthelastscheduleddayofattendancefortheweek,youareresponsible
tomakepaymentasagreed.Inthecaseofyourvacationorabsence,pleasepostdateyour
checkfortheupcomingdatedueandmakepaymentbeforeyouleave.
Iwillonlyacceptcashforpayment.LatepaymentsA$25latepaymentfee(perchild)
appliesforanypaymentnotreceivedontheFridaymorningbeforetheweekbegins.If
paymentisnotreceivedontheMondayoftheweekanadditional$10.00feeperdaywill
becharged.Yourchildwillnotbepermittedtoreturntochildcareuntilboththepayment
andthelatefeearepaidinfull.
Alatechargeof$5.00forevery10minutespastouragreedpickuptimewillbepayable
uponarrival.

ItisimportantthatIprotectyourchildbyensuringthatyourchilddoesnotleavemy
homewithapersonyouhavenotauthorizedonyouChildInformationCardtopickup
yourchild.Alsopleasetellmewhensomeoneelsethatyouhaveauthorizedonyou
ChildInformationCardwillbepickingupyourchild.Evenifitisanemergency,I
musthaveyourpermissiontoreleaseyourchildtosomeoneotherthanyou.Iwillneed

thepersonsnameandadescriptionofwhatheorshelookslike.Thepersonpickingup
yourchildwillhavetoshowmeapictureIDbeforeIwillreleaseyourchildfrommy
care.
Ihavetoassumethatbothparentshavetherighttopickupyourchild,unlessyougive
meacopyofacourtorderstatingotherwise.WewillneedtodiscusshowIshouldhandle
thenoncustodialparentwhoarrivestopickupyourchild.Withoutacopyofthecourt
order,Icannotrefuseaparent.IfIhaveacourtorderandanoncustodialparenttriesto
pickupthechild,Iwillimmediatelycallthecustodialparent.Ifthenoncustodialparent
leaveswiththechild,Iwillimmediatelycallthepoliceandreportthesituation.Iwillnot
placetheotherchildrenatriskinaconfrontationwiththenoncustodialparent.
Itisveryimportanttomethatyourchildarriveshomesafely.Therefore,Iftheperson
whoarrivestopickupyourchildappearsintoxicatedorotherwiseincapableofbringing
yourchildhomesafely,Iwillcalltheparentoremergencycontactpersonlistedonthe
ChildInformationCardtorequesttheirassistance.Ifthesituationoccursasecond
time,itwillbegroundsforterminatingmycareofyourchild.
Allchildrenshouldbetransportedtoandfromchildcareinacarseatorchildrestraintup
toageof8yearsoldand49.ForfurtherclarificationrefertotheArizonaLawregarding
childrenandseatbeltsandabidebythatlawforyourchildsafety.Iwillnotreleaseyour
childifthepersonpickingupyourchilddoesnothaveacareseatandyourchildfalls
intothecareseatrequirementagebracket.

ChildsHealth

TheStateofArizonarequiresthatanageappropriatehealthappraisalbeonfileforeach
childenrolledfollowingadmission,howeveryourchildcannotbeinitiallyadmittedto
daycarewithoutwrittendocumentationfromyourchildsphysicianornursepractitioner
thatatleastone(1)doseofDPTorDT,one(1)doseofTOPCorIPV,andtheMMR
vaccines,andHbCVvaccines,ifrequiredbytheageofthechild.Healthappraisalsshall
becertifiedbyyourchildsphysicianornursepractitionerandshallbeupdatedyearlyup
to the age of 5 in accordance with the recommended schedule for routine health
supervisionoftheAmericanAcademyofPediatrics.Forchildrenbelowschoolage,the
healthappraisalshallincludedocumentationoftherecommendationsofthedivisionof
publichealth,asdescribedbelow:

Parent/guardian must also complete a medical emergency card entitled Child
InformationCardandupdateasnecessary.
InaccordancewiththeArizonaStatelicensingpolicy,yourchildcannotbeadmittedto
daycarewithsymptomsofillnessasspecifiedbelow;unlesswrittendocumentationfrom
a licensed physician, or verbal (with written follow up) states the child has been
diagnosedandposesnoserioushealthrisktothechildortootherchildren.
Shouldyourchildhavesignsorsymptomsrequiringexclusionfromthefamilychildcare
homehe/shewillbeisolatedandtheparent/guardianorotherauthorizedpersonbythe
parentwillbenotifiedimmediatelytopickupyourchild.Therecanbenoexceptions
sinceillnessspreadsquicklyamongchildren.
PleasemakeotherarrangementsifyourchildissickandrespectmydecisionifIfeel
yourchildistoosicktobeinchildcare.Iamsympathetictothedifficultiesoftaking
timeoff,sodiscretionwillbeused.
Thesymptomsofillnessforpossibleexclusionshallinclude,butarenotlimitedtoanyof
thefollowing
A. The illness prevents your child from participating comfortably in the day care
environment,
B. TheillnessresultsinagreatercareneedthanIcanprovidewithoutcompromising
thehealthandsafetyoftheotherchildreninmycare,Or
C. Thechildhasanyofthefollowingconditions:
Temperature:Oraltemperature101degreesorgreater;axillary(armpit)
temperature100degreesorgreater;accompaniedbybehaviorchanges
orothersignsorsymptomsofillnessuntilmedicalevaluationindicates
inclusioninthefacility.Oraltemperatureshallnotbetakenonchildren
youngerthan4years(oryoungerthan3yearsifadigitalthermometeris

used).Rectaltemperatureshallbetakenonlybypersonswithspecific
healthtraining.
Symptoms and signs of possible severe illness (such as unusual
lethargy,uncontrolledcoughing,irritability,persistentcrying,difficult
breathing,wheezing,orotherunusualsigns)untilmedicalevaluation
allowsinclusion;
Uncontrolled diarrhea, that is, increased number of stools, increased
stoolwater,and/ordecreasedformthatisnotcontainedbythediaper
untildiarrheastops;
Vomitingillness(twoormoreepisodesofvomitingintheprevious24
hours)untilvomitingresolvesoruntilahealthcareproviderdetermines
theillnesstobenoncommunicable,andthechildisnotindangerof
dehydration;
Mouth sores with drooling, unless a health care provider or health
officialdeterminestheconditionisnoninfectious;
Rash with fever or behavior change, until a health care provider
determines that these symptoms do not indicate a communicable
disease;
Purulentconjunctivitis(definedaspinkorredconjunctivawithwhiteor
yelloweyedischarge),until24hoursaftertreatmenthasbeeninitiated;
viii. Scabies, head lice, or other infestation, until 24 hours after
treatmenthasbeeninitiated;
Tuberculosis,untilahealthcareproviderorhealthofficialstatesthatthe
childcanattendchildcare;
Impetigo,until24hoursaftertreatmenthasbeeninitiated;
Strepthroatorotherstreptococcalinfection,until24hoursafterinitial
antibiotictreatmentandcessationoffever;

erythromycin) to prevent an infection have been completed and a
licensedphysicianstatesinwritingthechildmayreturn;
Mumps,until9daysafteronsetofparotidglandswellingandalicensed
physicianstatesinwritingthechildmayreturn;
HepatitisAvirus,until1weekafteronsetofillnessorasdirectedbythe
health department when passive immunoprophylaxis (currently,
immuneserumglobulin)hasbeenadministeredtoappropriatechildren
andstaffandalicensedphysicianstatesinwritingthechildmayreturn;
Measles,until6daysafteronsetofrashandalicensedphysicianstates
inwritingthechildmayreturn;
Rubella,until6daysafteronsetofrashandalicensedphysicianstates
inwritingthechildmayreturn;

Unspecified respiratory illness if it limits the child's comfortable

participationinactivitiesorifitresultsinaneedforgreatercarethan

Nutrition:
Childrenarefednutritionallyonadailybasisbreakfast,lunchandasnackasrequired
through enrollment with the family and work place connection food program See
enclosedinformationandenrollmentformforyourchild.Cakes,cookies,andothernot
so nutritious food may be served during special events like birthday parties, and
holidays.Formulaisprovidedbytheparent/guardian,allotherfoodsandbeveragesare
providedbyme.
PottyTraining:
Pottytrainingshouldntberushed;itisimportantthatyourchildispsychologicallyand
physicallyreadyfortraining.HuggiesPullUps(orotherbrand)mustbeprovidedbythe
parent/guardianduringthistransitionperiod,noregularstyletrainingpantsorunderwear
willbeuseduntilyourchildmaintains2continuousweeksofbladder/bowelcontrol;of
course,ifyourchildregressesafterthis2weekperiodwewillassessthenextstep.
Transportation:
Itwillbeveryrare,buttheremaybeinstanceswhenyourchildmayneedtorideinan
automobileorvan.Iwillaskforwrittenpermissionunlessitisanemergency.

Supplies:
Parent/guardianwillprovidediapers,diaperwipes(ifyouwishtouseacertainbrand),
powder,anyointment(i.e.Desitinetc.),andbabybottles.Toeliminatethedailybundle
ofitemstocarryyoumaybringmeapackageofeachitemtoleaveatdaycare.Iwill
notifyyouifitemsarerunninglow.Allitemswillbemarkedwiththechildsname.
Parent/guardianwillprovideachangeofclothesonadailybasisorkeepachangeof
clothesatdaycareuntilneededreplacingasneeded.Aninfantmayrequiremorethan
onechangeofclothingdaily;pleaseprovideafewchangesofclothingbasedonyour
ownexperienceswithyourinfant.
Iwillsupplysleepingmats,portablecribs/playpens,andcribsheetsforyourchild.If
yourchildhasafavoritesleepingblanketorstuffedtoy,he/shemaybringthemtosleep
withincludingapillow.
Earlydropoff
Anycareneededpriortomynormalopeningtimewillneedbescheduledatleastbythe
Fridaypriortotheweekcareisneeded.Asaresulttherewillbeachargeof$3.00per15
minutes(orpartof)forcarepriortonormalopeningtime.Paymentforthisadditional
timeisduebytheFridaypriortotheweekcareisscheduled.Thisfeeisnonrefundable.
Thisincludesifyoudecidenottobringthechildearly.
Termination:
Parent/Guardian will give two weeks written notice, and two weeks full payment to
terminateyourchildsenrollmentinchildcareregardlessastowhetheryourchildis
present(withtheexceptionofthetrialperiod).Iftwoweeksnoticeisnotgiven,youare
stillfinanciallyobligatedforthetwoweeksofchildcarefeesandlatepayments;two
weeks full payment still applies when notice is given in conjunction with providers
vacation.
TrialPeriod:
Thereis atrial periodof4weeks fromthe date child carebegins.Ifthechildcare
arrangementsisnotmutuallysatisfactory,eitherpartycanterminatethisagreementwith
a1(one)daynoticeanymoneysalreadypaidarenonrefundable.
