    I, Shahid Nawaz , Registration Number: 04-3-1-024-2011 am aware of the fact that course registration should be
    performed in time and with extreme care. Once I am enrolled in a course I am bound to follow all rules and regulations in
    practice for the degree of BSME offered in the Institute.
    I will fulfill 80% attendance criteria in order to enable me to sit in the terminal examination of my registered courses.
    I will not apply for modification of my course registration information that I am registering today Wednesday, February 25,
    2015 under normal circumstances, and there is no ambiguity and uncertainity in my selection of courses.
    I admit that I am submitting -19 days late. I am submitting this form to my Head of Department/Course Coordinator for
