Mission Statement:
Our mission is to provide our community with high-quality,
individualized treatment to assist our clients in reaching their
maximum outcomes.
Philosophy:
Our philosophy is to serve and educate our clients by providing them
with a variety of hands-on treatment techniques as well as at-home
programs. We strive to provide our clients with one-on-one, cost
effective care to help them reach functional independence.
Goals of Department:
Provide the tri-county area with individualized treatment to help
them reach their goals
Integrate our services effectively with the hospital to treat our
patients with complete care
Treat patients of all ages to help them reach their functional
independence
Continue to improve our clinic by adding additional services
including occupational therapy and speech therapy in the future

Staff will include: 1 Director of Physical Therapy, 2 Physical Therapists,


2 Physical Therapist Assistants, 1 Physical Therapy Aide, and 1
Secretary/Receptionist
The Director of Physical Therapy will perform the management
responsibilities and physical therapy services as needed. The Director
of Physical Therapy will serve as the link to the hospital by reporting to
the medical staff director, who will then report to the board of trustees
of the hospital.
Target Population/Persons to be Served:
We will serve the citizens of the tri-county area. We will be available for
physical therapy services as the area consists of many farming, cattle,
and mining occupations. These occupations result in a higher
occurrence of overuse injuries; as well as, the potential for traumatic
farming and industrial accidents.
Types of Physical Therapy:
We will provide both inpatient and outpatient care. Types of physical
therapy include: orthopedic, wound care, neurologic, cardiopulmonary,
geriatric and pediatric treatments
Resources Necessary to Achieve Goals:
Personnel
o Director of Physical Therapy ($70,000): Will need one
person to fill this position. Will need at least 10 years
experience working as a physical therapist. Will need to be
comfortable in a leadership position. Will develop a budget
for the physical therapy department. Needs a strong
understanding of insurance and billing documentation. Will
monitor and supervise the physical therapy department
and report information directly to Medical Staff Director.
o Physical Therapist: Will have two physical therapists on
staff. Entry-level physical therapist will make $60,000.
Physical Therapists with at least 5 years experience will
make $65,000. Must be a graduate of a CAPTE- accredited
degree program in physical therapy and hold a state
physical therapy license. Must be able to evaluate and
make appropriate plan of care for each patient. Must be
able to lift at least 100 pounds and support up to 250
pounds. Must be able to transfer and assist patients up to
250 pounds. Must be able to maintain postures and
positions for several minutes at a time while delivering
patient care.
o Physical Therapist Assistant: Will have two physical
therapist assistants on staff. Entry-level physical therapist

assistants will make $42,000. Physical therapist assistants


with at least 5 years experience will make $45,000. Must
be a graduate of a CAPTE- accredited physical therapist
assistant program and hold a physical therapist assistant
license. Must work closely with physical therapists. Must be
able to lift at least 100 pounds and support up to 250
pounds. Must be able to transfer and assist patients up to
250 pounds. Must be able to maintain postures and
positions for several minutes at a time while delivering
patient care.
o Physical Therapy Aide ($13,000): Will have one physical
therapy aide on staff. Must be a high school graduate. No
prior experience is necessary. Will work to keep the
department clean by cleaning equipment and changing
linen. Will assist in the transport of patients. Must be able
to lift at least 100 pounds and support up to 250 pounds.
Must have a knowledge and understanding of system used
in scheduling patients.
o Secretary/Receptionist ($20,000): Will have one
secretary/receptionist on staff. Must have a knowledge and
understanding of system used in scheduling patients.
Comfortable with insurance and billing procedures. Must
have good organizational skills

o Towels, patient gowns, sheets, pillows, pillow cases, gloves,
electrical stimulation electrodes, ultrasound gel, athletic
tape, kinesiology tape, first aid supplies, cleaning supplies,
foam rollers, braces for patients, goniometers, tape
measures, reflex hammers, timers, business cards,
informational brochures
Major equipment
o Treatment tables, rolling stools, dumbbells, ankle weights,
therapy balls, wall pulleys, thera-band and tubing,
stationary bike, treadmill, arm bike, ice packs, electrical
stimulation machine, ultrasound machine, whirlpool,
paraffin treatment, hydrocollator hot pack and covers,
computer, printer, file cabinet, desks, chairs, paper, pens,
file folders, appropriate billing supplies, crutches, walkers,
wheelchairs, balance boards
Funding
o A citizen in the area had a great physical therapist while
recovering from a hip replacement; because of his great
experience he willed the hospital the money to create the
physical therapy department. After getting the department
started, we will rely on revenue to keep it running.

We will use a computer-based system for scheduling patients. This way
the schedule will be easily accessible for all members of the staff
through each computer in the department.

The Secretary and Director of Physical Therapy will be primarily


responsible for scheduling patients. Physical Therapy Aides can also
schedule when others are not available.

-Evaluate patients problems subjectively and objectively,
document data
-Assess a patients problems and create appropriate treatment

-Asses patients progress and make appropriate changes to
treatment plan, as needed
-Follow department policy by providing accurate and professional
documentation
-Provide initial evaluation with applicable disorders
-Submit progress/discharge reports to physicians and other
sources as required
-Direct and delegate to physical therapist assistant appropriately
-Instruct and educate patients and families in respective home
programs
-Keep up to date in continuing education requirements and
current research
-May function as clinical instructor for PT students

Dress code: Profession dress and appearance. Nametag to be
worn at all times. No artificial fingernails or fingernail polish.

may make contact with patients). Hair shoulder length and
longer secured back. No jeans or shorts. Men: slacks and collared
shirts. Women: slacks or dress pants, collared shirt or blouse.

Quality Assurance:
To monitor our personnel we will have each member of our staff
complete a performance appraisal reviewing how they felt they had
done throughout the year. This will allow each staff member to give an
honest evaluation of the year and point out his or her strengths and
weaknesses. Along with the performance appraisal, the director of
physical therapy will review up to 5 charts for each staff member to
check that things are being documented and charted appropriately.
These tools will be used to shape goals and make any necessary
changes for the upcoming year.

To monitor our services provided we will prepare a short survey for


patients to provide feedback for the department. This survey will be
given patients just before they are discharged. The secretary will
manage the survey so patients responses will not be changed by the
presence of the physical therapy staff. When surveys are complete
they will be taken to the director of physical therapy who will review
the results. At a year-end staff meeting, the results of the surveys will
be discussed. This will give the staff the chance to build on strengths,
make changes to minimize the weaknesses, and construct goals for the
coming year.

Areas of Development:
In the next 5 years we would like to include occupational therapy and
speech therapy. We want to provide therapy that treats our patients
total body and think that adding these additional services will make
this possible. We would like to grow our department for our patients
convenience and implement a variety of interventions for our patients.
