Case Presentation Form Assignment for Individual Counseling Sessions DIRECTIONS: Each counseling intern will present a videotape of an individual counseling session, The videotaped counseling session needs to show both the counselor and the client. For the presentation, you will prepare a document with copies for each class member. Include ALL of the following 7 items in your write-up: 1, Counseling intern’s name: Shelby Namba Name: Age: 15 Grade: 9" Gend Sexual Orientation: Bisexual Race/ethnicity: “ Religion: Unknown First generation: No Family Background: ~~ __isa middle child and lives with his mom, step dad, older brother (attends TJ as a junior), and young sister (in Pre-K)." also has an older, college-age sister who does not live at home. He has a pretty open and close relationshi with mom. He is fine with his older brother, younger sister, and step dad, but is not as close with them. I know very little about older sister and biological father. 3. Date of session: 5/4 Session #: 10+ Length: 25 minutes 4, Use $.0.A.P. (Subjective, Objective, Assessment, Plan) notes to capture the presenting issue: + Subjective: came in feeling very down and has felt this way for quite some time. He first voluntarily sought out help in the counseling office to talk about “emotional stuff/problems” within the first couple weeks of school. He consistently uses the word “down” to express how he feels. He speaks often of having “breakdowns” ~sometimes at school—where he describes how he becomes ‘overwhelmingly “emotional.” + Objective: The client's style has changed dramatically since I've first met him. His appearance has become more “emo” and trendy. He now confidently wears eyeliner (yay!). His hair is longer and frequently changes color. He appears physically healthy- a bit on the skinny side. He is usually well groomed. His energy seems low, which has become normal. He has frequently described his mood as depressed and emotional. He tends to lose focus and attention when difficult subjects are brought up (i.e. school, awkwardness with his father). + Assessment: The client’s current condition has not changed much for several years and appears to be mostly psychological. However, the most recent events in his life that have made his symptoms worse are environmental/sociological. For example, his father had lied about being terminally ill for several months (around winter break). Shortly after returning from break, his mother found out that the step-father had been lying and instead, has a number of mental illnesses. salso still struggling with virtual relationships and friends at school. From the information gathered since the beginning of the school year, the client has been diagnosed as clinically depressed. Since | have been working with him, he has tried anti-depressants and has also had a 504 accommodation plan put into place. | still believe his condition is significantly and negatively impacting his personal/social development as well as his academic achievement. ‘+ Plan: [have worked with since September. | have since referred him to a few outside resources (Sound Mental Health and YMCA CCORS). Before my focus was on a short term goal for ~ to able to go through his day without having to leave class to come see me in the counseling office. He is currently seeing a mental health counselor from SMH every Friday. Since | have referred out for his psychological issues, my goal is to redirect our focus on academics as he has become very behind in credits. This plan for our goals will be broken down as follows: @. Counselor: | will continue to provide individual counseling for von a weekly basis, but | have worked with his AVID teacher to focus our sessions on academics. | will connect with him about his depression and his worth with his SMH counselor. b, Advocate: | have had a total of four meetings with and his mother (who is extremely supportive). | am in constant contact with his teachers, mostly his AVID teacher, ¢. Scholar: | will continue to stay up to date with literature on childhood and adolescent depression. | have researched AVID as a program and have talked to both his AVID teacher and our AVID coordinator to learn more about the program This is in hopes of having a better understanding of what | can do to help him be successful in AVID and ultimately school 4. What multicultural factors are at play? How multicultural factors influence the client- counselor relationship? As far as ethnicity, | still don’t think that it is playing a role in the relationship | have developed with However, | still feel that “emo style and bisexual orientation are cont ig to his feelings of isolation and loneliness. We worked on being able to express his personality in his appearance. | have noticed a significant change in his physical appearance since the school year started—it is good that he is able to express himself in this way, 5. What counseling theory are you using in this session and what evidence do you have to support this? Before, many of my efforts to help have come from an Adlerian approach that ‘emphasized social interest. | tried to help him identify where and who he can find support in. 1 now find myself not really using a specific counseling theory and focusing more on academics; especially in this session. We work on study skills, organizational skills, and preparing him for his AVID class that subsequently helps him with the rest of his classes. Three things that I did well: 1.
