AFFIDAVIT OF DISCREPANCY

I,WELIE P. TUBASIS, of legal age, Filipino, married, and a

resident of Blk 21, Lot 21 St. jude Homes Gawaran Molino 7,
Bacoor Cavite, having been duly sworn in accordance with law,
hereby depose and state that:
1. In my Social Security System records, my first name
WELIE is erroneously spelled as "WILLY" instead of my
true and correct first name of "WELIE"
2. The correct spelling of my first name is "WELIE"; as
appearing in my Certificate of Live Birth;
3. I am executing this Affidavit of Discrepancy to attest to the
truth of the foregoing facts, to explain the discrepancy in
my SSS records, as regards my true first name in my
Certificate of Live Birth and to rectify my SSS records.
