I, ELISEA R. ALMOITE of legal age, widow, Filipino Citizen and with

Philippines, after having been duly sworn to in accordance with law, hereby
depose and state that:
1. I was awarded an Original Plan Of land MISCELLANEOUS SALES
covering a certain parcel of land situated at Rurban Code: 131102,
Resident section H, Baguio city containing an area of ONE
HUNDRED TWENTY FIVE(125) Square Meters, more or less and
logged as Msi-(1-2) 30-A approved on January 17, 2005 ;
2. I have a building erected over said lot and I also introduced
permanent improvements ;
3. I could not be able to pursue my

(MSA) until the issuance of a Certificate of Title because I was already

awarded a Certificate of Title over an adjacent lot covered by MSI-1-230 under Miscellaneous Sales Applications;
4. I am WAIVING AND TRANSFERING my RIGHTS AND ALL INTEREST
over the lot covered by Approved Plan MSI- (1-2) 30-A to my SON-INLAW CHRISTOPHER R. ALIPE , the husband of my daughter MYLINE
ALMOITE ALIPE and by this document he can continue processing of
my then MSA under his name;
5. I am executing this affidavit to attest to the veracity and truthfulness
of my foregoing statements and for all legal intents and purposes it
may serve.
