Purpose of the Assignment:
To provide a documented overview of the experiences encountered during the student's professional
training, co-curricular activities (e.g., professional organization activities such as brown bags, health fairs,
attendance at professional meetings), and extra-curricular activities (e.g., work, other organization
activities).
To connect the PharmD program outcomes to the professional training process
To allow the student an opportunity for self-assessment during the professional training process
To create meaningful opportunities for the student to meet with their College of Pharmacy mentor
To provide an opportunity for the student to give feedback to the college about the curriculum, specifically
related to the outcomes expected, so that continuous improvement can occur
Overview of the Portfolio:
The primary goals of the portfolio are to provide reflection on and documentation of professional
growth as a pharmacy student, using the Outcomes Expected Document as the underlying structure. A
written reflective report will provide a self-assessment of progress using the activities accomplished under
each domain as evidence of professional growth. Students are responsible for the selection of the elements to
support the experiences related to each domain. Documentation should come from course assignments, cocurricular, extracurricular activities, and internship.
Instructions:
Portfolio Elements (in this order):

A Cover sheet, your full name, your class year, your mentors name, and date.

Examples of materials that can support reflection on learning experiences and activities
include: (should not exceed 10 pages)
o Titles and course information for papers written
o Evidence of activities in which you participated documented in the CV/ Rsum and
described in the reflection. Other materials may also be provided.
o Pictures of professional activities (limited)
o Descriptions of projects completed

I.

Please see the appropriate section of the Internal Links portion of the College of Pharmacy web site for
several documents that pertain to the Portfolio assignment, this information can be accessed in the
following the sequence from the College of Pharmacy Internal Links page: scroll down to Student Links,
from the drop down menu click on Additional Links, then click on Student Portfolio documents.

II. The instructions contained in this document provide information only for the Reflective Report and SelfAssessment, which will be completed online. Hard copies of these must be saved and submitted for
review by your mentor. The Curriculum Vitae (CV) portion of the Portfolio should also be submitted as a
hard copy along with the hard copies of the Reflective Report and Self-Assessment.
III. Instructions for online submission of Portfolio Outcomes Assessment
This year you will be filling out the Outcomes Assessment tables electronically and saving to RxPreceptor
AND printing a copy to include in your portfolio.
1. Log in to RxPreceptor as you normally would. (Contact Chase Wilson at wilson@pharmacy.arizona.edu
if you have any trouble)
2. On the grey left-hand menu, there will be a button labeled "Surveys." Select it.
3. From the drop-down menu, choose "Pending Surveys."
4. You will have a survey entitled "Portfolio Reflective Self-Assessment." Choose it, and a pop-up window
will open. If you have some trouble with this step, please make sure any pop-up blockers associated with
your web browser are disabled.
5. Complete the survey by selecting the appropriate choice (Not Exposed, Novice, Intermediate, or
Proficient) for each of the competencies listed.
6. Important: Select, print this document to include a paper copy with your portfolio. Then select the
"Save and submit" option at the bottom of the page when you are done. If save and submit is not done
you will have to enter your responses again at another sitting.
IV. The Reflective Report done online is divided into three Domains and an Executive Summary. Each of the
three Domains has a Self-Assessment table. Based on your cumulative experiences (in and out of the
classroom) to date, complete the rating column of the Self-Assessment table, according to the instructions
provided, to the best of your ability. Specific instructions for use of the table are provided just above the
list of competencies. After completing the table, write your reflective comments in the free text section.
Reflections should comprise your feelings and reactions about the experiences that apply to this Domain.
Both positive and negative comments may be made. Reflections may also include commentary and/or
critique about how certain information or experiences were presented to you. This may apply to any of
your experiences, whether in or out of didactic class work. Reflections are not simply a list of what was
done. See the instructions in the verbiage immediately following the Summary Statement section for what
to write in that section. NOTE: a copy of what is available online is provided below so that you might fill in
prior to going online if you so desire.
V. Guidance on Length: Reflections for the Executive Summary and each Domain should be approximately
350 to 500 words.
VI. Definition of terms from tables
NE = not exposed = absolutely no experience or exposure to the given competence or activity.
N = novice = minimal, but not zero, experience with the given competence.
I = intermediate = moderate experience with the given competence such that you are familiar with it, but
not so as to function in providing it on an independent basis.
P = proficient = extensive experience with the competence, such that you could function to provide it
independently (although you may not yet be licensed to do so).
Note that the percentages of novice or proficient assessments do not influence the score given for the
Reflective Report. Rather, it is the quality, insight and thought given to the content of the reflections that
determines the score for the Reflective Report. This, along with the quality of the Curriculum Vitae (CV), is
what influences the total Portfolio score. The scoring form is provided in the portfolio section of student
links for your information.

VII. Submit the hard copy of your Reflective Report, Self-Assessment tables, and your curriculum vitae in an
appropriate binder to Estella Salgado in Room B307DD.
VIII. Submit the Reflective Report to Turn-it-in.com according to the instructions provided in the portfolio
section of internal links.
IX. Be very sure to save the three files (Reflective Report, Self-Assessment Tables, and Curriculum Vitae) on
your computer and on the colleges website in your folder. YOU are responsible for maintaining a copy.

The following are the Self-Assessment tables you will complete online and space to record your reflections if
desired. You may wish to print out the tables and fill in your ratings prior to going online. Your ratings on
these tables do not influence the grade received for the portfolio.
Domain 1: Patient Care - Dispensing medications and devices and ensuring appropriate
therapy and outcomes. The graduate will provide patient care in cooperation with patients, prescribers, and other
members of an interprofessional health care team based upon sound therapeutic principles and evidence-based data, taking into
account relevant legal, ethical, social, cultural, economic, and professional issues, emerging technologies, and evolving biomedical,
pharmaceutical social/behavioral/administrative, and clinical sciences that may impact therapeutic outcomes.

1.4 Implement the dispensing process for medication-related order
a. Select container for the drug product or determine if original packaging is safe and appropriate for the
product and consumer

Apply guidelines and standards of practice for preparation, storage, in-process quality control, and
administration of sterile dosage forms and enteral nutrition products in various pharmacy practice settings

Domain 2: Professionalism and management in health-systems. The graduate manages and uses
resources of the health care system, in cooperation with patients, prescribers, other health care providers, and administrative and
supportive personnel, to promote health; to provide, assess, and coordinate safe, accurate, and time-sensitive medication distribution;
and to improve therapeutic outcomes of medication use. In doing so, they provide patient care based on contemporary knowledge and
therapeutics in a compassionate, empathetic, and ethical manner. They must bring to the practice of pharmacy the necessary values,
attitudes, and behaviors to discern and manage the ethical issues of pharmacy practice. The graduate must demonstrate expertise in
