AFFIDAVIT OF UNDERTAKING
I, LUCILA C. EVANGELISTA-SIM, Filipino, of legal age, and a resident of 75
Lopez Jaena St., Galas, Quezon City, after having duly sworn to in accordance with
law, hereby depose and state that:

I am the registered owner of LJ Kitchenette located at Corte Real
Dormitory, Corte Real St. cor Solana St., Intramuros, Manila;
2.
I personally supervise and manage the operation of said business
establishment;
3.
In consideration of the tourist visa to be issued by British Embassy in the
Philippines, I undertake to return to the Philippines after the expiration of period
allowed therein;
4.
I am executing this affidavit to attest to the truth of the foregoing and for
the purpose of securing a tourist visa from the British Embassy.
