LABOR & POSTPARTUM DOULA REQUIREMENTS

Requirements for Mahdi Health Service Doula Certification

1. Verify you are a current member of Mahdi Health Service International
Membership number and date will be printed in packet information that you
will receive the first day of training.

Membership Fee is $35 (1) year membership and $70 (2) year membership can
be paid on our website.

2. Pay Certification Application Fee

Enclose processing fee of $50 USD. This payment is to be sent along with all your
paperwork once completed. Made payable in certified money order to: Mahdi
Health Service International

Online Payment via Website is available. Please indicate membership number

when paying online.

3. Attend MHSI Approved 3 Day, 4 Day, or 8 Week Training Course

Certification must be completed within 2 years of taking the training.

A copy of certificate of attendance signed by a Mahdi Health Service Instructor

must accompany application

4. Client Evaluations
Include (3) evaluation forms from clients that you have provided Labor and
Postpartum support
5. Submit Research Paper on one of the following not exceeding 500 words:
How your role as a birth doula will help to empower women of color
Describe how women of color can decrease the rate of Infant Mortality and
provide a solution

Describe the risk of intervention and how women of color can improve their
outcome
6. Document at least (3) births must include the following:
Client Confidentiality Release Form
(1) birth can be your own must be within 2 years
Birth Records for each birth attended
A cesarean birth can count for only (1) birth and must include the following:
Prenatal Information, Your presence at the birth, immediate postpartum support
and postpartum visit.
7. Develop and submit a resource list for clients that are local and national at least (30)
two form each category.
8. Attach a copy of your resume.
9. Read 3 Books from Reading List
A minimum of 3 books must be read and a report for each sent no more than
500 words
10. Complete one (1) of the following for Breastfeeding Requirements.
Proof of lactation consultant, breastfeeding peer counselor, certified lactation
counselor, or community breastfeeding educator training.
Proof of online lactation study at www.lactationtraining.com or
www.breastfeedingbasisc.org or attend Mahdi Health Service Basic
Breastfeeding for Doulas and provide proof of attendance.
Proof of participation in breastfeeding class or workshop. Submit class
outline and Breastfeeding Observation Form must be a minimum of 4 hours.
Denial of Certification

Certification may be denied to an applicant who falsifies information or if documents

are not complete.

Refund Policy

No refund will be given for membership, certification packet, application fee, or if

application is denied.

Extension

If you would like an extension for an additional 6 months please contact our office or
email us at mahdihealthservice@gmail.com and a request for extension form will be
emailed to you. Once received e will review and a payment of $30 must be submitted
before extension is approved.
