AFFIDAVIT OF EXTRAJUDICIAL SETTLEMENT

CARMENCITA B. TEJADA, after first being sworn according to law,
deposeth and sayeth:
That she is of legal age, widow, Filipino citizen, and with residence and
postal address at No. 43 Sto. Rosario, Baguio City;
That her husband, SALVADOR B. TEJADA, died intestate in Baguio City
on March 3, 2010 leaving the following described property:
PHILIPPINE SAVINGS BANK SAVINGS ACCOUNT BEARING
NO. 102-11100939-8 with a cash amount balance of P155,071.50
That the decedent left no known debts; neither has he left any
ascendants or descendants or any other heirs, except the herein affiant as
his wife.
WHEREFORE, and invoking the provisions of Rule 74, section 1, of the
Rules of Court, affiant does hereby adjudicate unto herself as the sole and
only heir the entire estate hereinabove described, subject only to the
liabilities to creditors, heirs, and other persons who might have been
deprived of their lawful participation in the estate, for a period of two years,
in accordance with Rule 74, Section 4, of the Rules of Court.
FURTHER, affiant sayeth naught.
