INSTRUCTIONS TO CANDIDATE

This form is ONLY to be used by FP Candidates whose applications were approved

from 1 Jan 2014 onwards and who have completed the Foundation Programme to
embark on the Professional Programme.

Candidate is required to be employed by an Accredited Training Organisation (ATO).

Please fill up the form and submit the signed copy together with all supporting
documents by POST to the following address:

Please ensure that the information stated below is same as your personal particulars
found in the Singapore QP Portal. If it is not, please update your personal particulars in
