As of June 28, 2015 I have reflected on my patients goals and their outcomes. For my

Patients first short term goal of adding thirty minutes of exercise twice a week, my patient has
been successful. My patient has kept a log of when he goes to the gym and what exercises he
participates in and for how long the duration is. Because a log isn't always an accurate indicator
we have had the trainer at the patients gym sign off when he has completed his workout regimen
for the day. For the intermediate goal of eating three meals a day and working in fruits and
vegetables my patient has also been successful. His lab values have came back within normal
limits as stated in the chart above. My patients overall health has improved and he says he feels
much better and healthier. This intermediate goal is still in progress and it is important that the
patient continues working on the success of the goal. For the third goal of losing thirty pounds
by next year my patient has already seen awesome results but its still in progress. So far in eight
weeks my patient has lost fifteen pounds, the weight started to fall off fast but the patient is
aware that the progress could slow down and its something that will need to be continually
worked at. For all of these goals weight and BMI can come into play to show the success of
each goal, the patient is very happy with his progress so far and he feels much healthier. Another
reflection of the success of these goals is the patients journal. The journal keeps track of his
thoughts, feelings, food intake including what each meal consists of and serving sizes, exercise
regimen including what exercises, when, duration, and weekly weigh ins. The patients progress
is moving right along in the right direction, he still has work to do to achieve his weight loss goal
and to maintain the exercise and diet goals. I am very happy with the patients progress so far and
look forward to keeping him motivated to continue to to work on the success of the goals.

My patient and I do not believe at this time that there is anything different that we would

do to make the goals more achievable. The patient and I are both happy with the outcomes so far
and the outcomes to come. The only thing I think that I could change would be the amount of
weight loss the patient could achieve, I think it could be more. However, I am happy with the
goal that we set so that the patient loses a healthy amount of weight in a healthy and doable time
frame.
