ISM Journal Entry 9/7/15:

Unfortunately, I just recently received all of my completed paperwork for my mentor


on Thursday due to his busy schedule, so I have not yet had the chance to meet or
experience anything with him. However, I have been in contact through email with
Dr. Blackwells secretary and himself, discussing arrangements for our first meeting,
and I plan to make up the hours that I have lost this week. I am going to meet with
Dr. Blackwell at UTMB on Monday to discuss what I will be experiencing, what to
wear, and some of the basics I will need to know before beginning.

ISM Journal Entry 9/14/15:


This week I met with my mentor twice both on Monday and Tuesday. Mondays
meeting consisted of planning out when we would meet, for how long we were to
meet, where we were to meet, and what my goal would be coming out of the
mentorship. We decided to meet once a week either Tuesday or Thursday for 2 and
a half hours at the Primary Care Pavilion at UTMB. We also discussed that I would
have the chance to meet with both my mentor and some of the residents that he
works with, in order to get different perspectives. Tuesdays meeting consisted of
first meeting with my mentor and second, going with the residents for the
remainder of the time to meet with their patients. I was able to see 4 different
patients all with diabetes with two of the residents. In each of the meetings with the
patients I was able to observe how the resident prescribed the patients medicine
depending on their progress and how the patients were feeling. I also was able to
observe the neuropathy test one of the residents performed on two of the patients.
And lastly to end the meeting my mentor and I discussed the final presentation and
how we were going to approach it. With these two meetings totaling to 4 hours I
have caught up on my mentor hours and look forward to meeting again this week.

ISM Journal Entry 9/21/15:


My meeting with my mentor this week consisted of meeting with 4 patients, talking
to a couple of the residents in the facility, and planning my presentation with my
mentor. The first patient I saw was a middle age woman with diabetes complaining
of knee pains we took her blood pressure and changed her medications the second
patient was a multiple sclerosis patient visiting the clinic for the first time and we
took her medications down refilled those she was in need of, and set up a
appointment with a new neurologist in the UTMB area. The last two patients were
young men need a physical for a new job. I also spoke with some of the residents
about college opportunities, experiences and different fields of medicine. And lastly
I took my picture with my mentor for the project and weebly website and also
discussed some of the information I would need in the future for my final
presentation.

ISM Journal Entry 9/28/15:

This week my mentor meeting included taking notes over normal ranges of blood
glucose, glycosylated hemoglobin, diabetic ketoacidosis, glycemic control, and
hypoglycemia and their effects on the person with diabetes. After taking notes we
met with one patient with diabetes. The patient came in after passing out for 12
hours straight, the conclusion the doctor reached was that it was a case of
hypoglycemia which we had previously learned about. The patient had taken their
insulin and not eaten after, causing there blood sugar levels to drop drastically
which resulted in a confused state. We also discussed different medications which
help the blood sugar levels and the affect exercise has on diabetes type 1 and type
2. We also made changes to my goal setting form which I had to re-submit to him
after making changes, we decided to add treatment plans along with prevention
methods to the final presentation.

ISM Journal Entry 10/5/15:


I met with my mentor this week for two and a half hours the first hour and a half my
mentor and I went over topics for the mid-term projects and found artifacts. The
second hour I went with the residents and saw two patients one that was new to the
clinic and the other was there to get refills on medication for their Chrons disease
and pain medication. The pain had recently increased due to the reconstructive
surgery they were having on their mouth. The resident refilled his medication and
stopped the other prescription so that the patient would not be receiving narcotics
from two different locations. The resident also performed a physical exam on the
patient to make sure nothing new had occurred.

Journal Entry 10/12/15:


Unfortunately, I didnt meet with my mentor this week due to the mid-term
presentation. However, I did work on my final product and notes for my mid-term
presentation in place of meeting with my mentor. I plan to make up hours for
meeting with him by going two days next week for two hours rather than the one
day that I usually would go.

Journal Entry 10/19/15:


This week I met with my mentor early in order to make up for last weeks missed
hours. For the first hour and a half we did research for diabetes and worked on
survey questions to finish my assignment. The second half of class I saw two
patients. One patient had gout and came in to get a refill on a prescription. The
patient was knew to the area so we also had to do a physical exam and extra test
for background information. The second patient I saw was a diabetic and was
complaining of frequent stomach aches and bowel movements. The resident gave
her a meal plan and told her it was most likely a virus of some sort and if it did not
resolve within a few days, she should return.

Journal Entry 10/26/15:


This week I met with my mentor, and saw two patients the first patient was there as
a diabetic patient and was coming in for a regular checkup. I watched the resident
preform examinations and refills the patients prescriptions. The second patient was
new to the clinic and had gout. He needed a refill on his prescription, but we also
had to preform examinations and take labs in order to provide a history in the
computer for his patient profile.

Journal Entry 11/2/15:


This week I met with one of the residents in the facility instead of my mentor. My
mentor was in Austin with his assistant. While there I interviewed one of the
residents who was in his second year of residency and also saw two of his patients
with him. Both of the patients were younger and there for regular checkups and
examinations, they were in good health. Secondly I put together my invitation and
finished the rest of the interview questions to email to my mentor and his assistant
to complete my interview assignment. I also spoke with my mentor about the time
date and location of my final presentation in order to complete the invitation
assignment.

Journal Entry 11/9/15:


This week I was unable to meet with my mentor due to he and I having schedule
conflicts. However, I did speak with his secretary to discuss dates and times for my
final presentation and i talked with him, his secretary and his assistant, in order to
complete the interview assignment. I plan to make up the hours next week when I
meet with him to discuss the final presentation.
