COURSE LEARNING ACTIVITY

CORE PROFESSIONAL COMPETENCIES ASSESSMENT (CPCA)


The purpose of this critical self-reflection learning activity is to assess behavioral competencies within six
(6) Core Competency Domains as established by Quality and Safety Education for Nurses (QSEN) and the
Institute of Medicine (IOM). Completion of the assessment will determine areas of strengths and areas that
need improvement.
Core Competencies in nursing are defined by QSEN as a standard set of performance domains in which it
is necessary for nurses to demonstrate proficiency to enter and practice professional nursing.
Competencies are defined in the EARN program as habitual, judicious use of communication, knowledge,
technical skills, clinical reasoning, emotions, values and self-reflection in daily practice. Competencies can
be technical and/or behavioral. A technical competency describes a technical skill. A behavioral
competency describes a certain behavior. It is an assumption that EARN students are proficient in technical
competencies, but may lack knowledge, abilities and attitudes for proficiency of domain specific behavioral
competencies.
DIRECTIONS:
1. WARNING! WARNING! WARNING! WARNING! This is a time intensive learning activity. It
requires an honest and accurate assessment of proficiency in practice. Careful reading and
understanding of the Core Competency Domain definition and behavioral competencies within the
domain will maximize assessment with the potential outcome of enhanced quality and safe care.
2. Use the CPCA tool to complete an assessment of behavioral competencies within each core
domain.
3. Rate ALL behaviors within each core competency using a 1-5 scale (1=limited competency, 5=high
level of competency).
4. Total ratings at the end of each core competency domain in the space provided.
5. After rating ALL behaviors within each core competency, provide one specific patient and/or
situation example for behaviors highlighted in blue. Note: DO NOT provide an example for all
behaviors within a Core Competency domain.
6. Develop a plan for improvement (using the table provided) for behaviors assessed at a level of less
than 4.

Customizes nursing care to patient needs and values every patient is different and care should be tailored to fit each patient individually. For example, I had a
patient in a nursing home setting. She had previously had a stroke and the residual affect was dysphagia and mild left-sided weakness. She was left-hand
dominate; therefore, the hand was very weak; Since she had dysphagia, I customized a nursing plan of care in which I helped her cut her food into smaller
pieces and made sure liquids were thickened so that she did not choke while swallowing the food.
Educates patient according to need: I also educated the nursing home patient according to her needs as it pertained to her having dysphagia. I began by asking
her what she thought dysphagia was and did she know what foods and liquids she should consume. I did this to assess her knowledge and what education I
needed to provide to her. I explained to her what dysphagia was. I instructed her that while eating, she should take small bites, eat slowly, maintain an upright
posture, and refrain from talking while eating so that she would not choke.
Demonstrates an awareness of the unity of body, mind, and spirit when providing care: It is believed that a person is not healthy unless the needs of their body,
mind, and spirit are fulfilled. I was able to address these aspects of health in a young patient that I cared for in a nursing home setting. She was there after an
automobile accident where she broke both legs. She was depressed because the other residents were either much older, permanently disabled, or cognitively
impaired; therefore, she was lonely and felt like she didnt belong there. We would listen to music on my I-phone and sing the lyrics. We also had many
conversations about God and the Bible because she missed not being able to attend her church meetings on Sundays.
Incorporates personal and professional value systems into providing diversity in care: I once cared for an elderly patient that would always get up in the middle of
the night to go to the bathroom; however, every time that the patient would get up, the patient would fall. It was mentioned by other staff that the patient needs to
be restrained at night time. I knew that this was not the best suggestion and definitely not the least restrictive option. I checked the patients MAR to make sure
there were not any nightly diuretics and found out that the patient was taking HCTZ in the afternoon. I obtained an order to change the dose to be given in the
morning. I also suggested that his room be the closes one to the nurses station, made sure the bed was closes to the floor and any obstacles that may contribute
to causing the falls be removed, adequate lighting, and a family member stay with the patient overnight. Within a couple of days, the falls were resolved.

Effectively communicates with patients/families by using standardized communication tools, communication principles and understanding of barriers to
communication: I believe that the patient is in control of their care. It is a collaboration between the patient, their family, and health care providers. I had a noncompliant diabetic patient. I was developing a care plan and included the patient and his wife in deciding the plan of care. I suggested that the patient check his
glucose at home at least 3 times a day, and monitor his carbohydrate and cholesterol intake. The patient informed me that he wanted to check his glucose only 2
times a day, and he planned to continue eating as he always has. He did not plan to live forever anyway. Even though, I did not agree with what he wanted for
himself, I could not tell him that he had to do it my way.
Effectively communicates by managing information, knowledge and technology to provide safe patient centered care and application of evidence to inform
practice decisions: When a patient is seen in the practice where I work, I verify against the patients chart, the patients name and date of birth, and why are they
being seen before any care is given. I also provide medication reconciliation to make sure that the patient is taking the correct medication, dosage, not taking any
duplicate medications, and there are not any contraindications.

Integrates best evidence to inform practice: I had a patient in the nursing home that had a feeding tube. Her daughter assisted her with her feedings, and I
happened to watch her check placement of the tube one day. The daughter injected air and listened for a noise. I informed the daughter in a polite way that we
no longer check for placement that way because of the lack of accuracy. I showed her how to aspirate the contents and perform the color test to make sure that
the placement was correct. The daughter was very thankful.

that a patient of his have a ICD replacement done. After the replacement, the patient was instructed to
remove the bandage on the second day, but she was instructed not to remove to bandaging strips until 2
weeks later. The patient removed the bandage and the strips which caused skin tears too. The daughter of
the patient called the clinic frantic and demanding to see the doctor as soon as possible. The doctor had a
full clinic and was not very happy about the situation. He used some profanity in voicing his frustrations;
however, I remained calm and professional knowing that he was just stressed. The doctor eventually
calmed down and agreed to work the patient in.

Minimizes risk of harm to patients and providers through both system effectiveness and individual performance : I currently work in a clinical setting; therefore, I
am limited in all that I can do to minimize risk when it comes to procedures that occur outside of the clinic at hospital settings; however, I have to make sure that
I accurately complete referrals for cardiac procedures, I make sure patients know what the procedure is and where and when they are to report to the hospital, I
have them stop medications that will interfere with testing and instruct them when to resume per the direction of the doctor, I have the doctor prescribe
medication if the patients are allergic to contrast dye, and I make sure they know when their follow-up appointment is.

Uses information and technology to communicate, manage knowledge, mitigate error and support decision-making: Patients and pharmacies often call to request
prescription refills or the pharmacy faxes or electronically transmits prescription request. Sometimes the medication requested has been discontinued by the
physician or it may be a different dose from what is listed in the MAR. It is my responsibility to consultant with the physician to make sure that the medication and
dosing is correct, labs are current and show adequate functioning of kidneys and liver, and read the physicians notes in the EMR to make sure that patients are
current on visits before I can approve any prescriptions.
Uses data analysis software: I dont have access to any data analysis software and have never used it.

Able to search, retrieve, manage and make decisions using electronic data from databases: We had to switch from ICD-9 to ICD 10 coding last month. I had to
do an INR for a patient that was taking warfarin because of Atrial Fibrillation. I could no longer use the ICD-9 coding for A-fib and did not know the ICD-10 code;
therefore, I utilized the internet to look up the ICD-10 code for Atrial Fibrillation and was able to list it on the patients super bill.
Evaluates health information for teaching patients/family: I performed INR test on patients that take warfarin or Coumadin. Depending on the range that the
patient is supposed to stay within, I have educated the patient and family on the dose of their medication and dietary intake. If they are testing at home, I instruct
the patient on how to test themselves and advise them to call in the results before 3 PM so that their dose can be adjusted if needed.
