PETITION
COMES NOW, Petitioner by himself, and unto this
Honorable Court, most respectfully states that:
1. Petitioner is a Filipino, 25 years of age, having been born
on October 06 , 1990 in Tagbilaran City, Bohol, single,
and presently a legal resident of the same;
2. Petitioner passed the 2014 Bar Examinations given last
October 2014 and has been admitted to the Bar last
April 27, 2015 with Roll No. 69696. Attached as Annex
A is a photocopy of Petitioners Certificate of
Membership in the Philippine Bar issued by the Office of
the Bar Confidant of the Supreme Court of the
Philippines;
3. Petitioner has been a resident of the Philippines since
birth for the last 25 years and holds office at Graham
Avenue, Cogon District, Tagbilaran City, where the
commission applied for is to be issued;
4. Petitioner is a member of the Integrated Bar of the
Philippines, Bohol Chapter;
5. Petitioner has paid his IBP dues as evidenced by O.R. No.
06524629, photocopy of which ishereto attached and
marked as Annex B and made an integral part of this
petition;
6. Petitoner is a bona fide member in good standing of the
Integrated Bar of the Philippines, Bohol
Chapter,

attached is a copy of his Certificate of Good Standing as

Annex C;
7. Petitioner has paid his Professional Tax for the fiscal year
2015 under Professional Tax Receipt No.4058471, a
photocopy is hereto attached as Annex D;
8. Petitioner has no pending administrative case as
evidenced by the Certification issued by the Office of the
Bar Confidant which is hereto attached as Annex E;
9. Further, Petitioner is submitting herewith three (3)
passport size colored recent photographs of himself with
his signature appearing at the bottom part of said
photographs, hereto attached and marked as Annexes
F, F-1 and F-2;
10.Petitioner desires to apply for the commission as Notary
Public for a period of two (2) years up to December 2016
and that he has all the qualifications and none of the
disqualifications to the Office of the Notary Public;
11.This is the first time that Petitioner applies for
appointment and commission as Notary Public for and in
the City of Tagbilaran and in the Province of Bohol and
he has not applied for commission before any
other jurisdiction.
WHEREFORE, it is most respectfully prayed of the
Honorable Court that, after due notice, publication and
hearing, Petitioners appointment and commission as Notary
Public in the City of Tagbilaran and in the Province of Bohol
please be granted in his name.
Other just and equitable reliefs are likewise prayed for.

allegations contained therein; and I certify that they are all
true and correct to the best of my own knowledge;

I further certify under oath that I have not commenced
any similar action before the Supreme Court, Court of
Appeals, or before any court, tribunal or agency concerned;
and should I learn of any similar cause of action filed before
any of the aforesaid bodies, I undertake to notify this
Honorable Court within five (5) days from notice.
