Patient Information:
Coleman Stucke is a 14 year-old boy who lives in Blaine Lake, SK with his mother. His mother, Sandra
Stucke, gained custody of Coleman following her divorce in 2005 with the father, Aaron Stucke.
Coleman also lives with his two other siblings, Karen and Bradley. His sister Karen is 18-years-old and
Bradley, his younger brother, is 11 years-old.
Coleman has been admitted to the hospital several times. First Coleman was suffering from the
following symptoms at the age of six: frequent urination, excessive thirst, increased hunger, weight loss,
tiredness, lack of interest and concentration while attending kindergarten. He was later diagnosed with Type I
diabetes. When Coleman was 12 years-old, he was administered penicillin for a bacterial infection in his throat.
Soon after he had difficulty breathing, hives, wheezing, dizziness, weak pulse, skin turning blue, diarrhea,
nausea, and vomiting. He had an allergic reaction to penicillin.
His mother had sent us information to add to his medical file from the school councillor. The school
councillors notes indicated that Coleman often worries about his relationships at school, he is always stressed
out about school work, he has high anxiety towards exceeding in sports, and he often struggles making
decisions under social pressure. He has shared with the councillor that he is often tempted to try smoking and
alcohol.
Colemans hobbies include playing video games, playing sports such as hockey and basketball, he
enjoys camping, and he is musically talented. He once had the lead position in the Northern Boys Choir
however he quit halfway through the year in 2014.

Being a part of the best medical team in the province, you have been specially selected to do a job. Your
job is to answer Colemans questions and help educate other youth like Coleman who struggle with staying
healthy by making an informative website.
You will be given articles to read about the questions below. Then, you will use a graphic organizer to
write facts along with your thoughts down. You will create a post on your website that will inform the public
about the problem and provide them with solutions on how to achieve a healthier life.
