AFFIDAVIT OF DESISTANCE
I, Karl Flores, 27 years old, single and a resident of Purok 4,
Georgetowm, Barangay. Rizal, Santiago City, after having been sworn in
accordance with law hereby deposes and states that:
1.

I am the same person executing this Affidavit of Desistance;

2.
I am the the same person, who was involved in a vehicular accident
last November 11, 2015 at Barangay Calao, Santiago City;
3.
I am executing this affidavit to express my intention of desisting
from further claiming any civil damages against Norman S. Obra, the driver of
the vehicle with plate no. GSS 247, whom I was involved in a vehicular accident;
4.
I am no longer interested in claiming any damages because of the
fact that we both acknowledged that it was really an accident and we also both
shoulder our own expenses as a result of the accident. As well as no one is to be
faulted regarding the accident;
5.
Based on the foregoing I no longer interested in claiming any
damages which I incur as a result of the accident and I hereby waive all my
claims, present and future, against Norman S. Obra;
6.
I am freely and voluntarily affixing my signature to this affidavit to
attest to the truth of the foregoing and to express my desire to desist from further
claiming for damages and to renounce and waive my claims, present and future,
against Norman S. Obra.
IN WITNESS WHEREOF, I have hereunto set my hands this 15 th day of
