MISSION STATEMENT
My mission it to provide quality client-centered care to individuals across the
lifespan to maximize functional independence, and live life to its fullest potential. Just as a daisy requires a collection of resources to blossom, so does an
individual. In providing care I will gather resources that will allow my clients
to thrive in the best possible setting, taking into consideration their volition,
goals, and environment. Growth is an important component to live; with that
being said, my mission will be to provide the best care to allow my clients to
grow.

LEADERSHIP PLAN
In my first year I will on take on a
Level I student so that I can establish my leadership role in the field
of occupational therapy.
In my fifth year I will demonstrate
competency in my scope of practice by organizing and leading a
parent workshop on managing
childrens behaviors. I also plan on
taking on a level II student and
advocating on Hill Day.
In my tenth year I plan on taking a
leadership role by organizing a
networking event with different
professionals in my setting. I also
plan on implementing a new program that integrates occupational
therapy for underprivileged teens
and children.

ARTIFACTS
In 2015, I joined and led the Healthy
Choices For Me program. The
programs focus is to fight obesity
through the focus of nutrition, physical
activity, and goal setting. This program
takes an occupation-based approach by
including activities on the schools
playground, as well as its focus on selfefficacy skills. By focusing on the
childs motivation, routines, habits,
and environment, self-efficacy can be
achieved. The ability to portray selfefficient behavior can be transferred to other beliefs an individual holds about their
capabilities in different aspects of his or her life. This leadership role is relevant to my
brand because I strive to empower my patients while taking into consideration their
motivation, routines, habits, and environment. Taking this holistic approach as a future
practitioner I plan on educating and empowering my clients to bring out their full
potential and achieve self-efficacy. This experience at Touro has shown me what it
takes to be a part of a successful team, such as open and constructive discourse in
addition to adaptability and dedication.
