of clients and constituencies.
How I will learn these tasks: I will act as an
advocate for residents and their families to
achieve discharge goals.
How I will measure what I have learned: Receive
input from agency supervisor, guidance of staff
nurses, physical and occupational therapy. Keep
weekly journal. Follow care plans and revise as
needed.

BEHAVIORS
24.Apply knowledge of human behavior and the social
environment, person-in-environment, and other
multidisciplinary theoretical frameworks in interventions
with clients and constituencies.
How I will learn these task: Attend care plan meetings, listen
to the residents wishes and be an advocate. Assist with
keeping family informed by telephone conference if they
are unable to visit frequently. Arrange discharge plans
home or to a continued long-term placement.
How I will measure what I have learned: If the resident is
happy and satisfied with the care and discharge plans that
are being made our goal has been met. Journal and seek
advice from agency supervisor.

BEHAVIORS
25. Use inter-professional collaboration as appropriate to
achieve beneficial practice outcomes.
How I will learn these task: Interview the St. Joseph of the
Pines residents to obtain their social, environmental, family
history on the intake forms. Assist in helping the residents
settle in and feel comfortable in their new surroundings.
How I will measure what I have learned: Successfully complete
assigned social work resident admission histories. Assist
with residents needs and request I will be able to
anticipate the needs of many of the residents as their
admission progress. Recognize and call the resident by
name in the hall, dining area or during activities.

BEHAVIORS
26. Negotiate, mediate and advocate with and on behalf of
diverse clients and constituencies.
How I will learn this task: I will work with diverse clients
while at SJOP and learn from the experience. Ex. I assisted
an elderly man that was anxious to pay a deposit for his
stay in rehab at SJOP. He had been unable to reach the
proper individuals in billing and the floor nurses did not
want to be responsible for the money.
How I will measure what I learned: I assisted the elderly
gentleman to contact the correct person in the billing
department and the proper collection of the bill occurred
and a receipt delivered to the resident. The resident was
pleased the issue was resolved.

BEHAVIORS
27. Facilitate effective transitions and endings that
advance mutually agreed-on goals.
How I will learn this task: Evaluate what is needed
within the SJOP facility and establish/develop an
agency project to assist the residents and staff
of SJOP improve the quality of residents comfort
and care.
How I will measure what I have learned: By
research, an out line, the submission of an
agency project proposal, and the completed
agency project presentation.
