ADDENDUM

Know All Men By These Presents:

This Addendum is being executed this 16th day of January 2013 at Quezon City, into by
and between:
LUCILYN M. VALENCIA, of legal age, Filipino, with resident and with postal 15-A 2nd
Street St. Ignatius Village, Quezon City, hereinafter referred to as the LESSOR.

-AndPOTENCIANO T. MEDINA, likewise legal age, Filipino and with residence and postal
address at Ground Floor, Sunshine Mall, FTI, hereinafter referred to as the LESSEE.

WITNESSETH, that:
WHEREAS, this pertains with the Contract of lease entered into by the above parties
which executed last 2007 and duly notarized by Atty. Dante V. Gil with docket no. 228; Page no.
46; Book No. X; Series of 2007;
WHEREAS, the parties executed an ADDENDUM of the above mentioned lease contract
mutually agreed upon that the same, upon expiration of its term, be extended into another year
period and the same has already expired last December 31, 2013 which the copy of which is
attached as form part of the said lease contract;
NOW, THEREFORE, fopr and in consideration of the foregoing premise, the herein
parties, have mutually agreed that the expired Addendum that had been expired last December
31, 2013, by virtue of this Addendum, be extended for another term of two (2) years and will
expres on December 31, 2016;
WHEREAS, other than the terms therein, the stipulated conditions, penal clause prohibition and
the manner of the use of the leased premise that governs their contractual relationship as set forth
in the herein attached lease contract shall be the same as what set forth thereinin the attached
leased contract they originally executed.
(For reference the copy of the lease contract i9s attached herewith as form part of this
Addendum).
IN WITNESS WHEREOF, the parties have hereunto set their hands and affixed their
signatures this 16th day of Janaury, 2014 at Quezon City.
