M.C.C.C. : I believe that I've obeyed the regulatory standards and all the rules and policies in the practice setting by making sure I come to
clinical setting on time and in good health (not having any flu that could affect my patients); I come in proper uniform with hand hygiene
always performed before doing any care (for prevention/infection control) and by maintaining confidentiality of my patient's information.

C.C. : In the begining you had problem focusing, which was very unsafe when patient care was involved.However, towards the middle of
the semester your ability to focus and care for your resident became much better. Your professional, legal and ethical practice improved a
great deal. Its very important that you begin each semester like your second half. This will require you to review daily before each clinical
and be able to demonstrated you knowledge very well from the begining until the end.

to learn and give the best possible care for my patients.

C.C. : Each clinical you were always aware of the your role and your responsiblity as a student nurse. With the periods of confusion that
was noted in the beginning you later improve by being more aware of your patient conditions when asked.

M.C.C.C. : I am aware of my strengths and limitations due to self-evaluation and from feedback of my clinical instructor and colleagues. I
received unsatisfactory for my midterm therefore I asked my clinical instructor for assistance and she helped me focus on the
areas/weaknesses I need to focus on.

C.C. : Student demonstrated ability to make changes during the clinical area. The first half she needed to be more knowledgable, organize
and follow instruction that were given.These were not always follow but when it was pointed out to her she took steps to ensure that
changes were made. She presented each clinical with a organize schedule and was able to answer critical thinking question much clearer
which also lead to her doing better assesment on her patient.

C.C. : It was reenforced each clinical to use the theory that was taught in class when caring for each of her resident. This was done much

easy reach and by always making their path clear.

M.C.C.C. : Whenever there's uncertainty, I would always ask my instructor, as well as the other health care providers for any clarifications.
For example, with the new patients I was assigned on, I asked the nurse how they normally like things the way they are or how they
normally like their medication to be given. I wanted to clarify in order to prevent any problems with my plan of care because both patients
are easily irritable.

C.C. : Student was always encourage to speack up while on unit and in post conference. Mini presentation was given to help student feel
much better in expressing her self.

M.C.C.C. : I've identified any unsafe situations for example when some food or some of their things were on the floor of my patient's room,
I made sure that everything is put away or cleaned in order to prevent any falls to occur.

C.C. : This area required a bit more renforcement at the begining of the clincial rotation but student soon learn and practice her role well.

Over all student demostrated her ability to make several changes during her clinical setting. Towards the end of the clinical student
demonstrated competent, and a safe practice as a professional strudent nurse role.

M.C.C.C. : I believe that I've improved compared than before regarding participation in team sharing and discussions during post
conference. I tried telling what happened during my day of clinical and what changes I will make.

M.C.C.C. : I make sure that I'm documenting on the right patient's chart especially now that I was assigned on two patients. No error of
documenting has occurred and I made sure that I am always on time with giving medications or checking their blood sugar and
documenting this afterward before I go for my break.

M.C.C.C. : I believe that I've reported relevant information to the appropriate personnel, for example telling the nurses and my instructor
that one of my patient doesn't wash their hands after toileting yet she is always rubbing her eyes or telling them when my patient's blood
pressure was low since she was taking hypertensive drugs.

M.C.C.C. : I believe that I've utilized communication techniques with the client especially because there's a language barrier therefore I
make sure that no misunderstanding is occurring. I've also given a patient's insulin and made sure that the charged nurse knows about it.

M.C.C.C. : I've used technology and my books to gather information when doing my drug cards (all the medications I have to give to my
patients) and research about my patient's diagnosis were all researched as well for further understanding.

M.C.C.C. : There's a language barrier with both patients since they speak Italian therefore understanding each other was definitely a
challenged. However, nonverbal gestures and simple words were used and I've even tried learning some basic and most commonly used
words in Italian for effective communication with my patients. Explaining procedures before starting any care is always done as well.

team members. Both patients are easily irritated and hence I try to be more understanding and patient when giving care instead of getting
irritated as well

M.C.C.C. : I am not sure if I have met this criteria

C.C. : More growth is needed but improvement has been made. Taking more time in the reviewing of skills prior to clinical will help much
more. Student have taken the step that is needed. Have been able to notice her response to her patient in the begining and at the end. In
week 9 was given an assignment which her patient had a language barrier, each week she came with different ways of approching the
resident and was able to see the change that was occuring, so in week 13 the patient was more open up and they both was able to
understand each other. The patent gave her hugs and thank her in English. This was a good use of her therapeutic knowledge and its effect
on the patient.

M.C.C.C. : I am aware of my beliefs, values and personal experiences and how this can have an affect on my practice. For example, I
believe that any type of abuse or harsh treatment is not acceptable especially when it is done by any nurse because I believe that nursing
is all about providing care and therefore those occurrences shouldn't be happening at all. That is why I always come in the clinical setting
with the mindset of making sure I can give the best possible care for my patients with an enthusiastic mind.

M.C.C.C. : I've interacted with my patients and even with other clients I wasn't assigned on. I've helped with the health care team with
feeding, washing other residents after bowel movement and help with dressing after the PSW have given the patient a shower.

team with patient care.

M.C.C.C. : I know that both my patients are Catholic and Italian and I understand that these can influence clients' choices and decisions

effective in communication, this was evident in several cases.

M.C.C.C. : I've provided effective client education for example I've taught and reminded my patient all the time to wash her hands after
toileting since most of the time she goes to the washroom by herself. I've also shown her how to use the hand sanitizer and made sure this

C.C. : During one occasion where student was instructed to wait to carry out a skill with the instructor she went ahead and perform the
skill without the instructor present.
During the conversation with the instructor student informed instructor that because she had done the skill the day before with the
instructor she assumed it was ok. After further discussion student understood the reason why it was important to follow instruction
carefully and voiced ways that she could have resolve any problems that may had occur. Knowledge base was acceptable and ability to
proble solve was begining to develop.

4. 2 Demonstrate leadership skills that foster group work and a positive learning environment.

M.C.C.C. : Any group work weren't done that showed leadership skills however I believe that I work well with my colleagues by always
trying to be as enthusiastic as possible to create a positive learning environment.

C.C. : Student took the initiative to help out on the unit with feeds, transport and help her peers with bed baths. These acts were
demonstrating her ability to foster group work and a positive learning environment. Staff have voice appreciation of how well she work with
the patients that she was not even assign to.

M.C.C.C. : I believe I need more development on identifying opportunities to improve health care service and nursing interventions by

C.C. : While student will need more opportunity to develop this area, this initial clinical experence provided her with a chance to begin the
process, each of her residents was cared for very well and if they needed any thing she knew how to request it from the staff.

M.C.C.C. : Peer feedback was done and I've accepted the feedback I've received from peers and my clinical instructor. I am always willing
to learn therefore I made sure I worked on the weaknesses that was point out (for example my confidence when giving care) to improve

C.C. : Student was good at writing out the care that is needed for each of the resident that was assign to her in the care plan but still need

M.C.C.C. : I made sure that my plan of care are holistic with resources used to make appropriate interventions for my patients. For
example, my patient is at risk for depression and for fall. With this in mind, I would stay with my patient since she's always alone in her
room and talk to her and make sure that her path is always clear with her walker near her.

C.C. : Due to student lack of self confident she has to be encourage to really think about the questions that she is being asked before
answering, her writing is very clear and indicate knowledge base but ariculating is sometime a problem.Encourge to spend more time
reviewing .

C.C. : In all your assign resident you have gone above and beyond to ensure proper care esp. the last four weeks when you were more

improvement. When something do not go smoothly for example when I wasn't on time with giving care and then going for break, I made
sure on the next day that I would manage/divide my time wisely with my two patients to accurately follow my organizational plan of care for
them.

C.C. : May need to give more examples of the various situation that you excericse your strengths and weakness. The fact that you wrote a
paper showing how you will improve in the areas of weakness and the examples that you presented could help for future reference.

communication was very hard due language barrier therefore became mad. I figured out everything she preferred with regards to her
morning routine therefore everything went smooth with giving her care the next day.

M.C.C.C. : Care plan was made for the assigned patient by finding out what priorities needed which was risk for fall and risk for infection.
Assessments were done and after the problem was identified, nursing diagnoses, expected outcomes, nursing interventions and
evaluation were made in order to set a goal for this problem to be solved.

M.C.C.C. : Self-reflection is always done after clinical in order to make any revision or changes if the plan of care to give a better and safe
and effective care for patients. For example, when it was my first time meeting my second patient, I had a hard time following my organized
plan of care for the day because of communication problems which resulted in a bad relationship in the beginning. Therefore, on that next
day I planned to change how I am going to approach my new patient and a better nurse-client relationship was definitely established by the
end.

C.C. : After the midterm student had a plan of care each week and was able to take instruction to change different things to ensure
success. She was then given two patient and was able to priorities the one that needed her attendion first.This area will still need more
improvement but with more practice student will be able to achieve her goal.

competent care for example by understanding and learning all about diabetes and blood glucose levels therefore I know beforehand why
and how I will give insulin to my patients.

M.C.C.C. : I've used prioritization strategies for client care for example, I would always check the care plan I made for my patients and
make sure I follow it in order to achieve the set goal made or to see any signs of improvement.

immedicately will be an area to improve in her prioritization strategies.

I continue to always come to clinical on time and with proper uniform. Basic rules such as maintaining confidentiality and performing hand hygiene are
always done as well. Overall, I believe that I've made improvements regarding my practice compare than my first few weeks in clinical. I've
understood and acknowledged my strengths and weaknesses and made sure that I really focus on this to ensure an effective and safe care for my
patients. Coming to clinical prepared with my readings and research done, along with the help of my instructor and colleagues, I believe that this really
help me become confident with my practice and communicate effectively towards my clients and the health care team.

Student started out with some difficulty during clinical setting which may be contributed to her lack of self confident, however, with continual
encouragement and assistance she has manage to improve to the point of taking on two resident for meds and care and have successfully completed
well. Will need continual preparation before clinical, stay focus, set priorities before clinical, have a plan and follow it while in the clinical setting. Put
your knowledge to practice remember patient first . Best of luck for the future. Always use the resources that are provided.
