BEHAVIORS
19. Collect and organize data, and apply critical thinking to
interpret information from clients and constituencies.
Task and learning activities: document the residents physical,
emotional, cognitive status as well as family support systems
and their social structure.
How I will measure: residents intake form, MDS form, SW
history, PT evaluation, my personal journal , input from field
supervisor.

BEHAVIORS
20. Apply knowledge of human behavior and the social
environment, person-in-environment, and other
multidisciplinary theoretical frameworks in interventions with
clients and constituencies.
Task and learning activities: one-on-one meetings with each
department and department chairperson. Allows this SW intern
a better understanding of the internal workings of the facility
and the SJOP team.
How I will measure: my ability to find my way around the SJOP
campus without getting lost, ability to utilize proper forms for
the residents needs, ask my supervisor when need guidance or
assistance.

BEHAVIORS COMPETENCY SEVEN


21. Develop mutually agreed-on intervention goals and
objectives based on the critical assessment of strengths,
needs, and challenges within clients and constituencies.
Task and learning activities: attend client care plan meetings;
discuss goals for d/c home; needs for successful return to the
community, support systems in place and those needed.
How I will measure: care plan meeting, resident focused care,
resident returns to ADL and a home care setting of choice w/wo
assistance.

COMPETENCY SEVEN BEHAVIORS CONTINUED


22. Select appropriate intervention strategies based on the
assessment, research knowledge, and values and preferences
of clients and constituencies.
Task and learning activities: interdisciplinary team meetings
w/nursing, PT, OT, SW, dietary, resident and family. Care plan
may reveal that resident is refusing Pt. Interventions could
include changing time of PT or therapists, a medication review
and asking the family to comfort and reassure the resident.
How I will measure: attend care plan meeting to maintain a
continuum of care. Discuss residents outcome and plans for
D/C. Talk with agency supervisor and keep personal journal.
