Upon recommendation of Luci Moreira, Director of the Postuguese School, I have the honor to offer you a
faculty appointment for the 2012 summer session. In addition to the responsibilities associated with your
teaching requirements, you are required to participate in, and sometimes lead, extra co-curricular activities
related to your School

no later than Mazch Ist, together with the following forms, which are all included:
+ Voluntary Insusance Enrollment Form (if applicable)
+ W-4 fosm (US. Citizens and Permanent Residents)
+ Diect Deposit form (only for those that have a U.S. bank account)
+ Housing and Board Information ~ should be submitted oa line (see attachment)
We are enclosing Form W-4, Employee's Withholding Allowance Certificate, Ifyou are a US. citizen, resident
alien, or green card holder, please consult with a tax advisor on how to complete this form. Please return the
completed and signed W-4 form with this contract. If you are a non-resident alien you ate not required to
complete this form, it will be provided to you upon your artival at the Language Schools.
Please visit the Human Resources Office or a Human Resources meeting within the first 3 days of the start of
your employment to complete an I-9 employment eligibility form. Enclosed you will find a copy of the U. S.
Department of Justice's list of documents that are acceptable as proof of employment eligibility in the United
States. You will be required to submit original documents when completing the 1-9 form.
Ti the next week you will be contacted by Intemational Student and Scholar Services (ISSS) at Middlebury
College. Please respond to their message as soon as possible, regardless of your citizenship and U.S.
immigration status. The message from ISSS will give you information and paperwork that will assist you through
the visa process, Please note that the visa process can take up to 4 months. If you ate already in the U.S. inApsil 13, 2012

Ifyou plan to bring a spouse/domestic partner or family o campus, the College will provide room and board
for them based on housing availabilty. U.S. tax law requires the College to tax employees on the benefit derived
from receiving “free” room and board for their spouse/domestic partner and “fice” room for dependent
childzen. ‘Taxes will be withheld fcom the first paycheck only. A stipend will be added to the paycheck of those
bringing a spouse /domestie partner and or dependent children to help lessen the employee's tax burden, If
you have any questions, please contact the Director of your School.
In May the College’s Tax Office will contact all foreign national employees who do not have a green card via
‘email asking for address, current and past visa as well as passport information via a secure online website. It is
your responsibility to comply with this request in a timely manner.
Please keep in mind that depending on how much you carn, you will be required (o file an income tax retoon
with the United States Internal Revenue Service and with the State of Vermont, by April 15, 2013. In February
2013 you will receive the necessary forms to fle your tax returns. Please make sure we have your correct
mailing address and email address on file
