SIADH
Sindroma ADH tetap ada tapi tidak adekuat fungsinya
Tidak ditemukan penyebab primary stimulus untuk
pelepasannya hypernatremia, hipovolemia
ADH berasal dari pituitary posterior atau dari kelenjar yang
patologis seperti neoplasma
Peningkatan ADH menyebabkan Volume ekstra vaskuler
ditahan, dan menyebabkan hyponatremia dan ekspansi dari ECF
Sebagai response terhadap ekspansi ECF, Na akan
diekskresikan
Urinary Na >30 mEq/L.
Terapinya : restriksi cairan
Hypernatemia
Hypernatraemia([Na]>150mEq/L)

Fisiologi Kalium
Kalium ion positif terbanyak di intraselular
Jangka pendek regulasi K ( dalam menit)
dipengaruhi oleh :
Insulin ( Na+ masuk ke sel melalui Na/H antiporter)
PH , K digunakan oleh tubuh untuk mengurangi kelebihan H+ di
ekstraselular dengan menggerakkan K+ keluar dari sel dan
H+ masuk sel ) (acidemia akan mencetuskan Hyperkalemia)
B-agonist ( Na / K ATP ase activity)
Konsentrasi Bicarbonate
Fisiologi Kalium
Pengaturan jangka panjang dilakukan oleh ginjal dan
aldosteron : menimbulkan ekskresi kalium
Kebutuhan kalium : 1-1.5 mEq/kgBB/hari
Hypokalemia
Kelainan elektrolit yang paling sering ditemukan (20% pasien yang
dirawat kadar K < 3.5 mmol/L)
Kadarnya di Intraselular 150 meq/L, di extraselular 3.5 5.0 meq/L
Mild 3.0 3.5 asymptomatic
<3.0 weakness, constipation
2.0 - 2.5 kelemahan otot, arrhythmias, perubahan ECG
< 2.0 -- ascending paralysis
Penyebab hipokalemia
Intake yang kurang
GIT Losses
Renal exkresi yang meningkat (excess mineral corticoids
&diuretics)
Pergerakan K ke intraselular pada kasus2 :
acute alkalosis
terapi insulin
hypokalemic periodic paralysis
Stress related chalechoamines
Terapi Hypokalemia
< 3.0 disertai perubahan gambaran EKG gelombang
T flat waves, gelombang U elevasi, prolonged PR
Terapi : melalui vena sentral 10-20 meq/jam untuk
dewasa
Hindari hiperventilasi, NaHCO3, B-agonists, insulin,
glukosa

The ECG patterns range from slight T wave flattening to the appearance of
prominent U waves, sometimes with ST depressions or T wave inversions.
Pola gelombang ini tidak spesifik untuk level tertentu dari kadar kalium

Calcium berguna untuk :
Kontraksi otot
Sekresi exocrine ,endocrine dan neurocrine
Pertumbuhan sel
Sekresi dan transport cairan dan elektrolit
Fisiologi Calsium
Bentuk Calcium
Berikatan dengan protein plasma terutama albumin dan
tidak difiltrasi oleh ginjal ( 40%)
Dalam bentuk terionisasi merupakan zat aktif, difiltrasi oleh
ginjal konsentrasi nya dipertahankan pada 2.0-2.5 mEq/L
(50%)
Calcium yang tidak terionisasi dan chalated dengan
phosphate , sulphate dan citrat(10%)
Penurunan kadar calcium dalam plasma bilangan menit
akan di kompensasi oleh kel paratiroid hormon
paratiroid akan meningkatkan reabsorbsi Ca dan
menurunkan ekskresi Ca

Hypercalcemia
Nilai normal 8.5 to 10.5 mg/dL 11-14 mg/100ml mild sampai
moderate hypercalcemia biasanya tidak ada gejala klinis
> 15 mg/dL life threatening
Keganasan hyperparathyroidism ( PTH), intoksikasi vit D
Intake yang meningkat renal damage, nephrolithiasis,
dehydration
Gelombang QT pendek, SV arrythmias
Volume resuscitation then volume + lasix
Tidak boleh thiazide diuretik
Pertahankan kadar Mg2+, K+
Calcitonin, mithramycin, dialysis
Hypocalcemia:
Hypocalcemia Ca2+ < 8 mg/dl
Neuromuscular
Perioral numbness
Kesemutan ujung2 jari tangan dan kaki
Hyperactive tendon reflexes
Carpopedal spasm
Muscle cramps

Terjadi pada : Malnutrition, alcoholism, redistribution (resp
alkalosis, refeeding/TPN)
Gejala klinis : kelemahan otot , rhabdomyolysis, hemolysis
Hypocalcemia bila dilakukan koreksi yang terlalu cepat
Hyperphosphotemia
Cell lysis, renal insufficiency
Gejala klinis disebabkan hypocalcemia
Terapi : phosphate binders, dialysis

Turut mempertahankan tonisitas plasma
Membantu absorbsi Na pada ginjal
Turut berperan dalam regulasi asam basa tubuh dengan
perannya sama dengan HCO3 Ekskresi asam melalui renal tergantung dari absorbsi
bikarbonat dan reabsorbsi chloride
