TargetPopulation
Oursmallfarmingcommunityismadeupofalargemajorityofcattlemenand
minersfrom3counties.Farmersinourareaareatahigherriskthanthenational
averagefortheoccurrenceofcerebrovascularaccidentsandrheumaticdiseases.Our
mainfocusisonmajortraumaticinjuriesandaccidentsthatcomealongwithindustrial
worksuchasfarmingandmining.
Weprovidenumerouscareoptionsincluding:

Toprovidewelleducated,highquality,evidencebasedcaretoourpatientswitha
focusontreatmentandconditioneducation.Wewantourpatientstobeawareofthe
howsandwhysofwhatwearedoingforthem,sotheycanfeelcomfortablewith
thecareweprovide.Wealsoemphasizetoourstafftheimportanceofgettingtoknow
eachpatientindividually.Bydoingthesethings,webelievethatthestaffandclients
maybemoreunitedintheefforttomakeimprovementsandreachsetgoals.
Goals
Inordertoensurethebestcarewepromisetogivetoourclients,wehaveabudget
forouremployeestocontinuetofurthertheireducationindifferentareas.Wealso
encouragethemtobeinvolvedinkeepinguptodateoncurrenttreatmentsand
information.Ourteamisalsoencouragedandwelcometocommunicateopenlyabout
anyconcernstheymayhave,sothatwemayworktogethertowardthebestsolutions.
Ourclientsmayalsowritedownanysuggestions,concerns,andcommentstoputina
commentboxatthefrontdesk.Wewillreadthosecommentsatourweeklymeetings
anddiscusssuccessesandanynecessarychanges.

individuals:1fulltimedirectorofphysicaltherapy,1fulltimephysicaltherapist,1

therapistaides.Becauseofthesmallsizeofourdepartment,wewillneedtohire
individualswhoarewelltrainedandhavespecializedinanareaofcaresuchas
neuromuscularormusculoskeletal.Wevaluegoodcommunicationskills,teamwork,
andapositiveworkingenvironmentamongourteammembers.Also,forthesafetyof
thosewhoenterourfacility,werequireallstafftokeepcurrentonimmunizationsand

FutureEndeavors
Inthenext5yearsorso,wewishtoexpandwithanoccupationaltherapyand
speechtherapydepartment.Althoughitisnotourmainpriorityrightnow,wewould
lovetoprovideasmanyservicesinthefutureaspossible.Wearealsosavingtoaddon
aroomforanaquatictherapypoolwithaconveyerbelt,andalsotheadded
functionalitytolowerandraiseourpatientsintothepool.Webelievethathavingapool
forourpatientstoutilizewillhelpeasetheirpainandincreasetheirmobilitywith

injuriesthatarecommoninourarea.Anotheradditionwewouldliketoprovidetothis
communityisahomehealth&hospicesection.Doingsowillallowustovisitpeoplein
