Medical Nutrition Therapy: Novato Healthcare Center, Marin General Hospital, Kaiser Permanente
Screen and identify patients of high nutritional risk requiring immediate nutrition intervention,
conduct comprehensive assessments of patient nutritional risk, develop and implement nutrition
care plans, interventions, and measurable goals to provide medical nutrition therapy, monitor and
evaluate problems, etiologies, sign and symptoms, and nutrition interventions, complete
appropriate documentation.
Conduct meal rounds to assess food quality, patient acceptance, eating and feeding concerns,
appetite, correct diet order implementation, availability of feeding assistance when needed,
