Definisi
A cleft palate is failure of the lateral palatal shelves
to fuse to each other. (Hand book of plastic
surgery)
Kegagalan fusi/ penyatuan dari ke dua sisi
palatum. (Schwartz)
Palatoschizis berasal dari bahasa yunani yang
berarti celah (schizis) pada langit-langit rongga
mulut (palatum) dan merupakan kelainan
kongenital

Anatomi
I. Palatum keras/ hard palate
Tulang: Maksilla (Prosesus palatina) +
Tulang palatina (Lamina horizontal)
Aliran darah: arteri palatina mayor
Saraf: Nervus palatina anterior

Anatomi
II. Palatum lunak
Lapisan fibromuskular melekat membentuk
lapisan dengan bagian palatum keras
Otot: tensor veli palatini (CN V), Levator
veli palatini (elevator primer), muskulus
uvulae, Palatoglossus, palatopharingeus
(CN IX dan X)

EPIDEMIOLOGI
Caucasians (kulit putih)=1.4 per 1000 live
births.
Asians=2.6 per 1000 live births.
African Americans=0.43 per 1000 live
births.

Perkembangan palatum dimulai

pd mgg ke 5 dan sempurna pd
mgg ke 12, dlm periode fetal.

Pada proses pembentukan palatum yg

normal terjadi 3 tahap :
1). Pertumbuhan
2). Naiknya kedua lempeng calon palatum
3). Peleburan menjadi satu
Apabila terjadi hambatan terhadap salah
satu atau lebih dr tahap tsb, maka
terjadilah palatoschisis.

Diagnosis
Pada umumnya, diagnosis ditegakkan saat
lahir, terutama melalui pemeriksaan fisik.

riw. ANC, ANB, medikasi, riw. keluarga.

Whole body, cari VACTERL
Status lokalis: cleft pd palatum durum/ molle/
keduanya, cleft bibir (+/-), unilateral/ bilateral,
uvula bifida (+/-)

 Hypernasal voice resonance VPI (Velopharyngeal

Inadequacy) koneksi antara kavum nasal dan oral
Pierre Robin syndrom (+/-) + mikrognathia,
glossoptosis, obstruksi airway

Px-an penunjang
Lab
Radiologi
Bila tidak disertai kelainan lain biasanya tidak
diperlukan
Bila disertai multipel anomali (+) Baby gram, foto
cranium, dll

Kemajuan teknologi dalam diagnosis prenatal

melalui USG dapat memperlihatkan cleft facial
saat bayi masih di dalam rahim.

KLASIFIKASI
Palatoschisis tanpa labioschisis/ disertai
labioschisis
Palatoschisis:

Uvula bifida
celah submukosa
celah palatum molle
celah pada keseluruhan palatum:
Komplit (total) palatum durum dan palatum molle,
dimulai dari foramen insisivum ke posterior
Inkomplit (subtotal)

Palatoschisis juga dapat bersifat unilateral atau

bilateral.

Menurut Veau, membagi cleft palatum

menjadi 4 kategori yaitu
1. Cleft palatum molle
2. Cleft palatum molle dan palatum durum
3. Cleft lip dan palatum unilateral komplit
4. Cleft lip dan palatum bilateral komplit

mencegah memasukkan tangan ke dalam
mulut
4. Diet cair, makanan lunak, sampai dengan 3
minggu, hindari penggunaan botol

Methods
Two flaps from the buccal mucosal areas were
inserted into the nasal mucosal raw surface at the
hard and soft palatal border to achieve nasal mucosal
pushback.

Forty-four children with cleft palates were

retrospectively studied to investigate the
velopharyngeal function, articulation, and
craniofacial morphology following primary
palatoplasty using the modified kaplan
method.

Results
The velopharyngeal function was evaluated as good
in 72.7% of cases, fair in 15.9%, and poor in 11.4%.
All four subjects who underwent the operation at 2
years of age or older were evaluated as good
Evaluated as poor included two patients who
undergone the operation at less than 1 year
Three who were treated at less than 1 year 6 months

Conclusion
The art of cleft palate repair has enjoyed
considerable development over many years
Although the controversies regarding the
timing and closure of a cleft palate seem to
have been resolved, with a consensus for
surgery being completed at 18 months
